Evidence-based medicine: botulinum toxin in focal hyperhidrosis

Abstract: All studies performed so far indicate that BTX-A is a safe and effective treatment for focal hyperhidrosis of the axillae and palms, for gustatory sweating, and for some other rare conditions associated with focal hyperhidrosis. Based on two large well-designed, double-blind, placebo-controlled study there is class 1 evidence for the efficacy of BTX-A in axillary hyperhidrosis, and class 2 evidence for BTX-A in gustatory and palmar sweating (classification according to the Quality Standards Subcommittee of the… Show more

“…The short-lived effect, with continued success depending on daily application, and skin irritations are the major drawbacks of this therapy [24]. Botulinum toxin is favoured by dermatologists and has obviously a positive impact on patients' quality of life [25,26]. However, botox injections require local anaesthesia, are expensive, and offer only temporary relief (usually between 3 to 6 months).…”

“…The most common form of focal HH is essential (primary or idiopathic) focal HH resulting from an over-activity of sweat glands mediated by sympathetically elicited attacks. Primary HH affects between 1% and 2.8% of the general population [2,3], and about 10% of these patients are affected by FH [4]. Sweating is a localised (e.g., in the face) or generalised adaptation syndrome, frequently concomitant to anxiety.…”

“…Topical glycopyrrolate, a quaternary ammonium anti-muscarinic compound, has been shown to signifi cantly decrease the incidence, severity and frequency of gustatory sweating and is well tolerated [ 83,84 ] . Botulinum toxin has been used for gustatory sweating, though in most literature its use is limited to unilateral surgical-related cases [ 85 ] .…”
