Evidence-Informed Patient Choice: Practical Issues of Involving Patients in Decisions About Health Care Technologies

Abstract: Evidence-informed patient choice involves providing people with research-based information about the effectiveness of health care options and promoting their involvement in decisions about their treatment. Although the concept seems desirable, the processes and outcomes of evidence-informed patient choice are poorly understood, and it should be carefully evaluated.

“…In proposing nomenclature for this review, the team has benefited considerably from the work of others 4,7,[11][12][13][14][15] and has collaborated closely with the USPSTF. 16 In addition, the development of IDM and SDM interventions is predicated on basic research, including decision analysis 7,[17][18][19] and risk communication.…”

“…5,[11][12][13] The purpose of this systematic review was to explore the extent to which IDM interventions have been tested and the potential outcomes empirically assessed. Although research on IDM in cancer treatment can inform IDM in cancer screening, this review was limited to the latter because people with diagnosed cancer differ in a variety of important ways from apparently healthy people who are facing decisions about screening.…”

“…Otro concepto que se presta a confusión es el de elección basada en la evidencia (evidence based patient choice) (49)(50)(51). En este caso, el paciente recibe una información basada en pruebas de efectividad de al menos dos tratamientos de un determinado trastorno, aportando al proceso un "cierto input", de manera que la decisión es "en cierta forma compartida" entre paciente y profesional (50). Sin embargo, esta concepción es tan amplia que bien podría admitir que el propio médico tomara la decisión (29) 2.3.…”

“…42 These enhanced deliberative skills could then be later called into play at new decision cross-roads. 35,36,[43][44][45][46][47][48][49] This teaching strategy would take advantage of the interactivity of computer-based or webbased media. For a particular decision situation, a patient decision aid could first present core objective clinical information about the disease and its treatment options, then invite the viewer to select simulated "case descriptions" of three socially matched "patients" (say, Patients X, Y, and Z), who all are of the same sex, age, and ethnocultural background as the viewer.…”
