Evolution of continent ileostomy

Abstract: Continent ileostomy can be defined as a surgical procedure that facilitates planned intermittent evacuation of a bowel reservoir through an ileostomy. It was devised by Nils Kock in 1969. Subsequently, continent ileostomy (or Kock pouch) became a viable alternative in the management of patients who had traditionally required an end ileostomy. Kock pouch appeared to provide substantial physical and psychosocial benefits over a conventional ileostomy. The procedure became popular until ileal pouch anal anastomos… Show more

“…Around 50–70 cm of small intestine is required for the construction of a low‐pressure compliant Kock pouch, and the technique has evolved and undergone a few modifications since its first description . Kock's original continent ileostomy did not have an intussuscepted valve; instead the efferent limb of the pouch was relatively long as it passed through the abdominal wall, the intention being for the rectus abdominus muscle to act like a valve as it contracted .…”

“…Later, continence was maintained through a nipple valve formed by retrograde intussusception of the efferent intestinal limb into the pouch. (72) Despite continence, several surgeons reported the valve slippage and detachment from the abdominal wall. Further modifications were developed in order to staple the nipple valve by Kock, Stein and Fazio.…”

“…The technical evolution of proctocolectomy is one of the most interesting chapters in surgery. Over the past seven decades, the choice of methods in the historical sequence has changed from conventional ileostomy (IS) over continent ileostomy (CI) to restoration of the normal defaecation pathway by ileal pouch–anal anastomosis (IPAA) 1 . CI was introduced in 1969 in an attempt to preserve the functionality and body image of the surgically treated patient requiring proctocolectomy 2 , and the procedure was adopted widely thereafter 3 .…”
