Endodontic treatment failure may occur due to different causes such as persistence of bacteria, root canals that are poorly cleaned and obturated, improper coronal seal (leakage), and untreated canals (missed canals). The main reason for endodontic failure is the presence of some species of bacteria inside the root canal system such as Enterococcus (E.) faecalis. Those bacteria are more resistant to disinfection agents, causing a persistent intra-radicular or extraradicular infection. The current review aims to compile all the current studies concerning Enterococcus faecalis as a dental root canal pathogen that causes endodontic failure. In this systemic review, two databases, PubMed and Google Scholar, were searched using specific inclusion and exclusion criteria. Among 2943 studies, only 11 met the inclusion criteria and were included in the review for further analysis. The 11 studies give prominence to the high distribution of Enterococcus faecalis within the root canal system. These studies investigated different aspects of Enterococcus faecalis, including its prevalence, resistance mechanisms, characteristics, express survival genes, and treatment. The compiled data observed that most of the studies highlight Enterococcus faecalis as the primary pathogen associated with endodontic treatment. It has characteristic proprieties that make it capable of escaping disinfection means. Furthermore, clinical trials are required to examine E. faecalis and may provide valuable information about novel microbial detection methods to decrease the number of E. faecalis within the root canal system.

The Leap protocol combines the newest and most active association of reciprocal-rotary instrumentation, aluminum insert ultrasonic, and diode laser. The reciprocal-rotary technique consists of Niti instruments used in different motions depending on the stage of the treatment and promotes the balanced cutting effectiveness and reduced risk of fracture files. Moreover, it creates a centered and tapered shape. The aluminum tip for the ultrasonic unit delivers agitation, emulsification, and cavitation of antiseptic solutions that improve the cleaning. The dye and laser use cause the ablation and final disinfection. This paper describes the case of a patient with asymptomatic apical periodontitis in an upper first right molar submitted to endodontic treatment performed with Leap protocol. After conventional procedures, the root canals were prepared by using reciprocal-rotary instrumentation techniques. Between each file a new solution was placed and activated with the ultrasonic handpiece. At the end of instrumentation, the root canals were aspirated, dried with paper points, the indocyanine dye was placed into the root canals and, DaVinci laser was used for 30 seconds. The gutta-percha cones matching the final Niti file were used coated with AH Plus sealer. A periapical radiograph was taken to verify obturation and its anatomical complexity. Six months after the procedure, the patient was asymptomatic, and the radiographic examination showed healthy periradicular tissues. It is concluded that the use of Leap protocol in conventional endodontic treatment was effective, suggesting that this therapy may provide additional benefits to patients when compared to the conventional technique.

A BSTRACT Background and Aims: Rising effects of temperature due to laser use during root canal disinfection may harm periodontium and alveolar bone. Therefore, the aim of this study was to estimate the surface root temperature of lower incisors throughout the application of different power levels and times of an 810-nm diode laser. Materials and Methods: Sixty single-rooted extracted human lower incisor teeth were selected and chemomechanical preparation was performed. Specimens were irradiated using 810-nm diode laser at 1.05, 1.5, and 1.95 W power settings and two periods of time 20 and 60s, in a continuous wave (CW) mode, without water spray. Specimens were divided into three main groups ( n = 20). Each group was subdivided into two subgroups ( n = 10). Then, the peak temperatures at the middle and apical regions of the root surface were registered using a thermocouple. Results: Temperature rise of root surface at all the selected output powers was below 7°C. The highest temperature value was obtained in the apical region at 60s when the root canal irradiated at 1.95 W output power. Conclusion: Diode laser is safe for use as a root canal disinfectant. Time of exposure to laser irradiation has an effect on the temperature difference at different output powers.
