Exacerbation of immunoglobulin G4‐related inflammatory abdominal aortic aneurysm after endovascular repair

Abstract: A 78-year-old male was admitted to our hospital with lumbar pain and was found to have an abdominal aortic aneurysm (AAA) and femoral artery aneurysm (FAA). Initially, the patient underwent endovascular aneurysm repair (EVAR) for the AAA and aneurysmectomy for the FAA. The FAA was diagnosed by histology as immunoglobulin G4-related disease (IgG4-RD). The preoperative serum IgG4 level was within the normal range, although a slight serum interleukin-6 (IL-6) elevation was observed. Four years later, the AAA-sac … Show more

“…Considering the unstable vital signs and the presence of an adequate iliac proximal and distal neck, we ultimately opted for a faster and less invasive endovascular treatment to reduce perioperative and intraoperative risks [ 27 ]. However, it is worth noting that some reports suggest that endovascular procedures can lead to more clinical symptoms and postoperative complications when compared to open surgery, such as persistent inflammation, periaortic fibrosis, aneurysm expansion, or even recurrent aneurysm rupture [ 28 30 ]. Maheshwari et al presented a case in which IgG4-RD was identified as the etiology of an external iliac artery aneurysm.…”

“…Por otro lado, al evaluar la terapia con corticoides en pacientes con FRP idiopática debe tenerse en cuenta la remodelación arterial, porque la dilatación aórtica progresiva puede ocurrir durante el tratamiento con esteroides. Aunque tales cambios suelen ser subclínicos, se han reportado casos que requirieron tratamiento quirúrgico por rotura, como sucedió en nuestro paciente (7,9,10).…”
