Excellent Clinical Outcomes and High Retention in Care Among Adults in a Community-Based HIV Treatment Program in Rural Rwanda

Abstract: A program of intensive community-based treatment support for ART in rural Rwanda had excellent outcomes in 24-month retention in care. Having committed to improving access to HIV treatment in sub-Saharan Africa, the international community, including country HIV programs, should set high programmatic outcome benchmarks.

“…For example, in rural Rwanda, intensive community-based support for HIV treatment, which included such features as free antiretroviral therapy, daily visits from community health workers, a monthly food package, a transportation allowance, and the availability of patient support groups, resulted in 92 percent of patients remaining on treatment during a two-year period, with few deaths and excellent clinical outcomes. 18 Optimizing HIV Therapy Alongside efforts to expand HIV testing and improve the continuum of HIV care, researchers are working to identify ways to improve antiretroviral drugs and the provision of antiretroviral therapy. For example, trials are under way to study longer-acting formulations of drugs that might be given monthly or even less frequently, to minimize the number of doses a patient must take.…”
