Background: Frailty assessment is a critical approach in assessing the health status of older people. The clinical tools deployed by geriatricians to assess frailty can be grouped into two categories; using a questionnaire-based method or analyzing the physical performance of the subject. In performance analysis, the time taken by a subject to complete a physical task such as walking over a specific distance, typically three meters, is measured. The questionnaire-based method is subjective, and the time-based performance analysis does not necessarily identify the kinematic characteristics of motion and their root causes. However, kinematic characteristics are crucial in measuring the degree of frailty. Results: The studies reviewed in this paper indicate that the quantitative analysis of activity of daily living, balance and gait are significant methods for assessing frailty in older people. Kinematic parameters (such as gait speed) and sensor-derived parameters are also strong markers of frailty. Seventeen gait parameters are found to be sensitive for discriminating various frailty levels. Gait velocity is the most significant parameter. Short term monitoring of daily activities is a more significant method for frailty assessment than is long term monitoring and can be implemented easily using clinical tests such as sit to stand or stand to sit. The risk of fall can be considered an outcome of frailty. Conclusion: Frailty is a multi-dimensional phenomenon that is defined by various domains; physical, social, psychological and environmental. The physical domain has proven to be essential in the objective determination of the degree of frailty in older people. The deployment of inertial sensor in clinical tests is an effective method for the objective assessment of frailty.

Background: Exergaming is a fun, engaging, and interactive form of exercising and it may help overcome some of the traditional exercise barriers and help improve adherence by older adults providing therapeutic applications for balance recovery and functional mobility. The purpose of this systematic review is to summarize the effects of exergames in older adults’ mobility and balance. Methods: The PRISMA guidelines for systematic reviews were followed. The following databases were searched from inception to August 2019: Cochrane Central Register of Controlled Trials, MEDLINE, EMBASE, PEDro, CINAHL and INSPEC. We selected randomized controlled trials that assessed the effects of exergames on balance or mobility of older adults without neurological conditions, in comparison to no intervention or health education. Two review authors independently screened the trials titles and abstracts and identified trials for inclusion according to the eligibility criteria. Trial selection presented an almost perfect agreement between the authors regarding the interrater reliability (kappa = 0.84; p<0,001). Then, a descriptive analysis of the quantitative data was performed to summarize the evidence. Meta-analysis was carried using Revman. Random effects model was used to compute the pooled prevalence at 95% confidence interval. Results: After screening 822 trials, twelve trials comparing exergames with no intervention were included. A total of 1520 older adults participated in the studies, with mean age of 76±6 years for the experimental group and 76±5 years for the control group. Three studies found significant improvements in balance based on center of pressure sway and Berg Balance Scale scores. Three studies found improved mobility based on the timed up and go, 30-second chair stand, and 8-foot up and go test. Conclusions: Exergames improved balance and mobility in older adults without neurological disorders. High quality studies with standardized assessment protocols are necessary to improve evidence.

IntroductionFibromyalgia (FM) is a chronic musculoskeletal condition characterised by reduced quality of life and severe limitations in daily living activities. Considering the wide spectrum of symptoms and the ineffectiveness of a single pharmacological approach, the latest clinical guidelines recommend non‐pharmacological therapies as both an alternative and a better‐tolerated approach. Several studies have been conducted to determine the effectiveness of non‐pharmacological therapies in the management of FM.AimsThrough a literature review, this paper aims to describe the different complementary therapies and investigate their potential sustainability and effectiveness on FM symptoms in the short and/or long term.MethodsWe searched the PubMed and Google Scholar databases using broad search terms up to June 2022, to identify all types of study designs restricted to human subjects on non‐pharmacological therapies in FM.ResultsRecent evidence demonstrated that physical activity is the mainstay of therapeutic management, highlighting the relevance of walking as the best method of exercise in FM patients. Nevertheless, adherence to physical activity remains fraught with obstacles that could be overcome with a multimodal and multidisciplinary approach involving a wide range of passive therapies. The effectiveness of passive non‐pharmacological therapies remains however unproven in the long term. They can be therefore suggested as ‘adjunct’ or ‘bridge’ therapy to improve adherence to physical activity.ConclusionTo conclude, FM management requires a multimodal and symptom‐based approach, guided by the predominant bothersome symptom on the one hand, and the preferences of each patient on the other hand.
