Expanding opportunities for chronic disease prevention for Hispanics: the Better Together REACH program in Pennsylvania

Abstract: BackgroundHispanics in Lebanon and Reading, Pennsylvania, experience high levels of socioeconomic and health disparities in risk factors for chronic disease.
