Background Although nurses are crucial to ensure patients’ peaceful death in hospitals, many nurses experience various ethical conflicts during end-of-life care. Therefore, research on nurses’ entire ethical decision-making process is required to improve nurses’ ethical decision-making in end-of-life care. This study aimed to identify Korean nurses’ ethical decision-making process based on their moral sensitivity to end-of-life patients. Methods In total, 171 nurses caring for terminal patients responded to the survey questionnaire. To measure the participants’ moral sensitivity and ethical decision-making process, we used the Korean version of the Moral Sensitivity Questionnaire and Nurses’ Ethical Decision-Making around End of Life Care Scale. Finally, multiple linear regression analysis was used to investigate the effect of moral sensitivity on nurses’ ethical decision-making. Results The mean of moral sensitivity was 4.8 ± 0.5 (out of 7), and that of ethical decision-making was 4.6 ± 0.5 (out of 6). Among the sub-dimensions of ethical decision-making, the highest score was in perceived professional accountability (5.2 ± 0.5), and the lowest in moral reasoning and moral agency (3.9 ± 0.6); the score of moral practice was 4.4 ± 0.7. In the multiple linear regression model, moral sensitivity (β = 0.852, p < .001), clinical department (β = − 7.018, p = .035), ethics education (β = 20.450, p < .001), job satisfaction (β = 5.273, p < .001), and ethical conflict (β = − 2.260, p = 0.031) were influential ethical decision-making factors. Conclusions This study revealed a gap between nurses’ thoughts and practices through the ethical decision-making process. They failed to lead their thought to moral practice. It also implies that moral sensitivity could positively affect nurses’ ethical decision-making. To make nurses morally sensitive, exposing them to various clinical cases would be helpful. Additionally, ethics education and clinical ethics supporting services are valuable for improving nurses’ ethical decision-making. If nurses improved their ethical decision-making regarding end-of-life care, their patients could experience a better quality of death.

Background The theory of planned behavior is a conceptual framework of recent studies to identify and explain nurses’ intentions to care for patients with emerging infectious diseases. However, correlations between behavioral intentions and variables that explain them have been inconsistent in previous studies. The influence of new variables might be considered in this case. This study aimed to determine moderating effects of ethical nursing competence on nurses’ intention to care for COVID-19 patients in hospitals dedicated to infectious diseases based on the theory of planned behavior. Methods A cross-sectional survey was conducted. Data on intention to care for COVID-19 patients, perceived behavioral control, attitude toward the behavior, subjective norm, and ethical nursing competence were obtained from 190 nurses in three hospitals dedicated to infectious diseases in South Korea. The moderating effect of ethical nursing competence was analyzed using model I of PROCESS Macro. Ethical considerations: This study was approved by the Institutional Review Board of Ulsan University Hospital, South Korea. Written informed consent was obtained from each subject. Results The ethical nursing competence was a significant moderator in the relation between perceived behavioral control and the intention to care (B = 0.36, t = 2.16, p = 0.032). Ethical nursing competence did not have a significant interaction with attitude toward behavior or subjective norm. Conclusions This study showed that the higher the ethical nursing competence level, the greater the effect of perceived behavioral control on nurses’ intention to care for COVID-19 patients. Promoting ethical nursing competence is necessary for nurses who would take care of patients at the frontline of the infectious disease pandemic. Nursing managers should include ethical nursing competence in the assessment of nurses’ competence and design educational programs to enhance ethical nursing competence for efficient nursing staffing during a pandemic.

The aim of this study was to develop an Ethical Nursing Competence Self-rating Scale for Clinical Nurses. Methods: A scale-development study was applied that comprised eight stages of DeVellis. The scale verification involved a convenience sample of 423 nurses from September to October 2019 at three general hospitals located in Korea. The content validity, factorial structure validity, item-convergent/discriminant validity, known-group validity, convergent validity, internal consistency reliability, and test-retest reliability of the Ethical Nursing Competence Self-rating Scale for Clinical Nurses were evaluated. Data were analyzed using exploratory and confirmatory factor analyses, Pearson's correlation coefficient, Heterotrait-monotrait (HTMT), Cronbach's ⍺, and intraclass correlation coefficient. Results: Exploratory and confirmatory factor analyses yielded five-factors. Known-group validity was demonstrated by clinical experience and nursing ethics education experience. Convergent validity was demonstrated using measures of defining issue. Internal consistency reliability and test-retest reliability were found to be acceptable, as indicated by a Cronbach's ⍺ of .70~.85 and an intraclass correlation coefficient of .72~.89. Conclusion: The Ethical Nursing Competence Self-rating Scale for Clinical Nurses is a new instrument that comprehensively measures the aspects of ethical behavior, ethical decision-making and action, ethical sensitivity, ethical reflection, and ethical knowledge. It consists of 20 items scored on a 4-point Likert scale. The validity and reliability of the scale were verified. These findings indicate that the instrument can be applied in clinical practice, nursing education, and research.
