Experiencing uncertain HIV treatment delivery in a transitional setting: qualitative study

Abstract: In the absence of reliable and sustained treatment access, information and support, PLHIV absorb the anxieties of system failures. This uncertainty led to an individuation of "treatment". PLHIV adopted rationing strategies to mediate their anxiety, energy and hope. This predominately resulted in varying forms of disengagement and neglect for social change. It is likely that this has significant negative implications for the promotion of HIV treatment advocacy and anti-stigma efforts.

“…There were similarities between our respondents’ reports with discrimination experiences with those of PLH in other post-socialist countries (Amirkhanian et al 2003; Novotny et al 2003; Rhodes – Simic 2005; Štulhofer et al 2008; Bernays – Rhodes 2009; Bernays et al 2010). The similarities illustrate the disempowering effects of HIV/AIDS stigma especially in denial of access to work either by being fired from workplaces or not being hired for jobs.…”

“…For example in Croatia in 2009, out of 26 antiretroviral formulations registered in the EU, 14 were available to Croatian patients. There are also occasionally irregular supplies of antiretrovirals and the level of stigma towards MSM and patients with HIV/AIDS is high [13], [14]. Although there are not many patients in need for treatment and the health care insurance system is universal and free of charge for the individual, there is pressure from health care authorities to save costs.…”

“…Within the HIV literature there has been some focus on feeling with discussions of the importance of hope and hopelessness (Bernays, Rhodes, and Barnett 2007;Kylm€ a, Vehvil€ ainen-Julkunen, and L€ ahdevirta 2001). The causal links between poor adherence and hope is posited such that certain environments do not engender hope or longterm planning which, in turn, weakens self and community regulation of risk (Bernays and Rhodes 2009). But hope itself as a concept has been left relatively uninterrogated; considered simply as 'a positive expectation of the future' (Bernays, Rhodes, and Barnett 2007).…”
