Expositions récréatives de 8 patients aux nouvelles drogues de synthèse obtenues sur Internet : à propos de 3,4-méthylènedioxypyrovalérone (MDPV) et de méthoxétamine (MXE)

Abstract

Résumé -3,4-méthylènedioxypyrovalérone (MDPV) et méthoxétamine (MXE) font partie des nouvelles substances dites « récréatives » facilement obtenues sur Internet. Neuf cas d'exposition à ces substances sont rapportés pour 8 patients, l'un d'entre eux ayant consommé les 2 produits. Méthodes : Plusieurs approches analytiques sont décrites par CLHP/UV-BD après extraction alcaline éventuellement suivie d'une réextraction acide, par CPG/SM après extraction alcaline et par CLHP/SM-SM après simple précipitation des protéines. Résultats : Les concentrations plasmatiques mesurées dans les prélèvements d'admission sont, pour la MDPV, égales à 45 μg/L et inférieure à la limite de quantification et, pour la MXE, comprises entre 122 et 366 μg/L sauf 1 cas inférieur à la limite de quantification. Les concentrations urinaires sont comprises entre 0,3 et 165 mg/L selon le produit et les patients. Plusieurs métabolites supposés sont mis en évidence comme la N-deséthyl-MXE et la O-déméthyl-N-deséthyl-MXE. Les symptômes décrits sont tachycardie, hypertension artérielle, agitation, convulsions et coma. Deux patients ont nécessité des soins de réanimation. L'évo-lution était favorable pour tous. Conclusion : Si la MDPV vient d'être classée comme stupéfiant en France, la MXE bénéficie encore d'une absence de statut. L'augmentation des données clinico-biologiques concernant cette substance doit conduire à une réflexion sur un classement futur comme stupéfiant. Mots clés : Méthylènedioxypyrovalérone, MDPV, méthoxétamine, MXE, CLHP/UV-BD, CLUHP/SM/SMAbstract -Among the legal high drugs easily obtained by internet, 3,4-methylenedioxypyrovalerone (MDPV) and methoxetamine (MXE) were observed in 8 patients who needed hospitalization after ingestion for an isolated experience (n = 4) or collective recreational use (n = 4). Clinical symptoms were hypertension, tachycardia, agitation, hallucinations, and coma for 2 patients. Methods: Plasma and urine analyses were performed by classical HPLC/UV-DAD and GC/MS screening approaches, both with quantitative measurements by either HPLC/UV after back-extraction at acidic pH and/or UPLC/MS/MS after single protein-precipitation purification. Results: MDPV levels (n = 2) were 45 μg/L and below the limit of quantification for plasma, and 2.3 and 0.3 mg/L for urine samples. MXE plasma levels were between 122 and 366 μg/L for 6 patients and below the limit of quantification for one patient. Urine levels (n = 6) were between 2 and 165 mg/L. Two supposed metabolites of MXE were identified as N-desethyl-MXE and O-demethyl-N-desethyl-MXE. Conclusion: While MDPV is now a controlled substance in France, MXE has no status. These observations should be taken into account for a possible future registration.

A case of fatal idiosyncratic reaction to the designer drug 3,4-methylenedioxypyrovalerone (MDPV) and review of the literature
