Recent findings implicate alterations in glutamate signaling, leading to aberrant synaptic plasticity, in schizophrenia. Matrix metalloproteinase-9 (MMP-9) has been shown to regulate glutamate receptors, be regulated by glutamate at excitatory synapses, and modulate physiological and morphological synaptic plasticity. By means of functional gene polymorphism, gene responsiveness to antipsychotics and blood plasma levels MMP-9 has recently been implicated in schizophrenia. This commentary critically reviews these findings based on the hypothesis that MMP-9 contributes to pathological synaptic plasticity in schizophrenia.Key words: synaptic plasticity/glutamate/extracellular matrix Matrix Metalloproteinase-9 in Animal Studies Matrix Metalloproteinase-9 Expression and Activity in the BrainMatrix metalloproteinase-9 (MMP-9) belongs to a family of zinc-dependent proteases, mostly secreted as inactive pro-enzymes, activated outside the cell upon proteolytic cleavage. MMPs degrade extracellular matrix constituents, other proteases, growth factors, and extracellular domains of transmembrane proteins, such as cell adhesion molecules and receptors. As a result, MMPs participate in remodeling the pericellular microenvironment and cell-signaling via either the release or processing (to reveal their binding domains) of cell-surface receptor ligands.

Razvoj sofisticiranih tehnika detekcije strukturnih i biohemijskih abnormlnosti, u poslednjih nekoliko decenija, odigrao je važnu ulogu u preciznijem sagledavanju brojnih bioloških alteracija kod pacijenata uopšte. Ova činjenica je od posebne važnosti istraživačima u oblasti neuronauka, zbog često nepoznate i/ili multifaktorijalne etiopatogeneze neuropsihijatrijskih poremećaja, te svaki deo spoznaje kompleksnog "algoritma" ima svoje značajno mesto. U poslednje vreme veliku pažnju istraživača privlače eksploracija imunoloških abnormalnosti kod obolelih od shizofrenije, kao i citokinima posredovani mehanizmi koji predstavljaju osnovu za niz hipoteza iznesenih na ovu temu [1][2][3]. U daljem tekstu fokusiraće-mo se na nekoliko najvažnijih teorijskih pravaca koje se tiču inflamacije i imunoloških promena zapaženih u shizofreniji. Jedna od teorija je da je infekcija majke tokom trudnoće (naročito tokom ranog perinatalnog perioda) značajan faktor rizika za dete za razvoj shizofrenije i drugih neurorazvojnih oboljenja [4,5]. Epidemiološke studije su pokazale da infekcija majke virusom influence tokom trudnoće povećava rizik za shizofreniju deteta za 3-7 puta [4,6]. Pored virusa influence i brojni drugi infektivni činioci su povezani sa povećanim rizikom za SCH medju kojima su i varicela zoster virus, polio, difterija, rubela itd [5]. Medjutim, patološki mehanizam odgovoran za povećan rizik za shizofreniju kod dece nakon izloženosti majke infekciji još uvek je nedovoljno jasan. Takodje otvoreno je pitanje, na koji način infekcija majke dovodi do citokinskog disbalansa kod fetusa; da li je u pitnju delovanje infektivnog činioca na specifične receptore koji dovode do produkcije i oslobadjanja različitih medijatora zapaljenja u periferni majčin imuni sistem koji onda prolaze placentu i ulaze u cirkulaciju fetusa [6,7], ili potencijalni iz- Kratak sadržajPoslednjih par decenija pažnju istraživača sve više privlače imunološke abnormalnosti uočene kod obolelih od shizofrenije. U prvom delu teksta prikazan je kratak pregled pojedinih teorijskih konstrukata predloženih u cilju boljeg uvida u povezanost shizofrenije i imunoloških alteracija, u drugom delu je razmotren uticaj citokina na pojedine neurotransmiterske sisteme i finalno izneta su zapažanja dobijena iz istraživanja u realnoj kliničkoj situaciji kao i pravci budućih istraživanja.

One hundred and three psychiatric inpatients (74 men) were assessed for a wide spectrum of autoantibodies including antinuclear, antismooth muscle, antimitochondrial, antiDNA, anti-phospholipid, anti-cardiolipin IgG and IgM, antikeratin, rheumatoid factor, antithyroperoxydase, antigliadin IgA and IgG, antitransgutaminase, and antiendomysium antibodies. Four groups of patients were considered separately, including 47 with schizophrenia, 23 with schizoaffective disorder, 16 with bipolar disorder and 17 patients with other different psychiatric diagnosis. Forty one healthy, age- and sex-matched blood donors were used as a control group. There were no significant difference in the prevalence of the different autoantibodies between patients (N = 103) and controls except for antigliadin IgG (30.1 vs 9.8 respectively, p = 0.01). Presence of autoantibodies was influenced by age but not by sex or treatment. As for diagnosis categories, patients with bipolar disorder presented significantly more autoantibodies than the three other categories and controls. These results point out a possible autoimmune activation in at least a subgroup of psychiatric patients especially amongst those suffering from bipolar disorder.
