Expression of cyclooxygenase-2 in the mucosa of the gastroesophageal junction in patients with Barrett’s oesophagus – the results of ablation therapy with argon plasma coagulation and laparoscopic Nissen fundoplication

Abstract: A b s t r a c t Introduction: Barrett's oesophagus (BE) is a result of replacement of the mucosa of the distal oesophagus by metaplastic columnar epithelium. The role of cyclooxygenase-2 (COX-2) and chronic inflammation in the progression of BE toward adenocarcinoma of the oesophagus is emphasized. Argon plasma coagulation (APC) allows for effective ablation of metaplastic mucosa. It is believed that effective suppression of gastric secretion with proton pump inhibitors (PPI) promotes squamous cell re-epitheli… Show more

“…Signs of GERD constitute the principal indication for surgery in patients with this condition. Laparoscopic Nissen fundoplication, including plasty of hiatal hernia, remains the technique of choice and usually is not associated with any greater difficulties [ 4 ]. However, qualification of patients from this group for surgical treatment is frequently questionable.…”

“…To date there are no univocal, consistent recommendations for the treatment of BE [12]. Despite the growing interest in the management of this condition in most countries the only recommendation for patients with BE is endoscopic surveillance with the aim of early recognition of cancer.…”

“…Some authors also recommend resection procedures for patients with HGD. Numerous endoscopic and surgical methods of abnormal epithelium removal are currently being developed [12]. Antireflux surgery as prophylaxis against reflux of gastric contents eliminates the underlying cause of BE [12, 14].…”
