Extended-spectrum β-lactamase (ESBL) in Danish clinical isolates of Escherichia coli and Klebsiella pneumoniae: Prevalence, β-lactamase distribution, phylogroups, and co-resistance

Abstract: Compared to 2003 data the ESBL prevalence in Denmark has increased significantly. In the ESBL-producers, reduced susceptibility towards both gentamicin and ciprofloxacin was seen among 43% E. coli and 55% K. pneumoniae, leaving clinicians in these cases with only a carbapenem for the treatment of serious infections. Part of this study was presented at the 20(th) European Congress of Clinical Microbiology and Infectious Diseases, abstract P-1617.

“…In Denmark and many other European Union countries, the most prevalent CTX-M variant among human isolates is CTX-M-15 (60%), followed by CTX-M-14 (12%) and CTX-M-1 (11%) (29). CTX-M-15 is rarely detected in animals (2%) (6), thus indicating that the majority of human infections caused by CTX-M-producing E. coli are not attributable to zoonotic transmission.…”

“…the association of bla CTX-M genes with ciprofloxacin and amikacin resistance [44,45]. Co-resistance, as expected, was highest in K. pneumoniae [45], with a single, multi-resistant clone being dominant among the isolates. The co-resistance pattern of the 2 E. coli types was also very different.…”

“…ESBL production was screened for by cefpodoxime resistance analysis and the double-disk synergy test, with disks containing cefotaxime and ceftazidime with or without clavulanate (30). MICs for 18 antibiotics (amoxicillin-clavulanic acid, ampicillin, apramycin, cefotaxime, ceftiofur, chloramphenicol, ciprofloxacin, colistin, florfenicol, gentamicin, meropenem, nalidixic acid, neomycin, spectinomycin, streptomycin, sulfamethoxazole, tetracycline, and trimethoprim) were determined by broth microdilution using a Sensititre system (Trek Diagnostic Systems Ltd., United Kingdom) according to the manufacturer's instructions and Clinical and Laboratory Standards Institute guidelines.…”
