Extending the Horizon in Chronic Heart Failure

Abstract: Background-The long-term impact of chronic heart failure management programs over the typical life span of affected individuals is unknown. Methods and Results-The effects of a nurse-led, multidisciplinary, home-based intervention (HBI) in a typically elderly cohort of patients with chronic heart failure initially randomized to either HBI (nϭ149) or usual postdischarge care (UC) (nϭ148) after a short-term hospitalization were studied for up to 10 years of follow-up (minimum 7.5 years of follow-up). Study end p… Show more

“…In the USA, the Organized Program to Initiate Lifesaving Treatment in Hospitalized Patients with Heart Failure registry20 revealed 60–90 day event rates of 36.1% in HF with reduced ejection fraction (HFrEF) and 35.3% in HF with preserved ejection fraction (HFpEF). The high rate of primary outcome may be due to old age and the high prevalence of co‐morbidities 19. This is also similar to mortality rates in the same region from a previous study 21…”

“…In Australia, Stewart et al . 's randomized controlled trial of home‐based intervention showed a 1 year event rate (death or unplanned readmission) of 74% in the usual care arm and 57% in the home‐based intervention 19. In Australia, Robertson et al 7.…”

“…In a study on the health-economic performance of cardiac rehabilitation for 10 years, the cost required to gain an additional life-year is US $806 among patients undergoing cardiac rehabilitation, which is lower than the US $1,073 required for patients without cardiac rehabilitation (Table 64). 335 In another study where the cost-effectiveness of a 14-month exercise training program in patients with stable chronic heart failure, the cost required to gain an additional life-year was higher in patients with cardiac rehabilitation than in those without it. These findings indicated that longer exercise training is more cost-effective.…”

“…Several studies have shown that HF programs involving a multidisciplinary team result in improved clinical outcomes of patients. 90 Such programs improve treatment adherence rates, ensure self-care management and facilitate access to supportive services. The prevalence of HFPEF increases with aging, 3 and these programmes may provide benefits particularly in the elderly population.…”
