Extent of surgery and survival in early lung carcinoma

Abstract: The finding that limited resection is inferior to lobectomy has relevance to the hypothesis that chest X-ray screening may lead to the identification of clinically unimportant lung carcinomas, which have been termed pseudotumors. This hypothesis, known as overdiagnosis, is the only alternative to the conclusion from four existing randomized trials that chest X-ray screening leads to an improvement in lung cancer cure rates. However, if extent of surgical resection is the major determinant of survival in Stage … Show more

“…These patients have the greatest hope for cure following standard procedure of resection. Survival vary, with reports on 5 year overall survival of 36% to 84% for pathologically proven stage IA and IB diseases [4,14]. Mean values on overall survival at 5 years of 67% for postoperative pathological stage IA and of 57% in stage IB are reported, with a difference of 8Á38% between stage IA and IB.…”

“…Lobectomy and pneumonectomy are superior to atypical resection [4]. It is reported that the worse outcome with atypical resection is not only influenced by an increased local failure rate but mainly by perioperative morbidity and mortality.…”

“…Chemotherapy shows the only marginal benefit and the impact of treatment on the survival and quality of life of patients with NSCLC remains controversial. 61,62 Due to the limitations of controversial therapy, the development of new anti-NSCLC agents is highly desired. The potent cytotoxicity of Superstolides A and B against NSCLC make both molecules attractive synthetic targets.…”

“…The potent cytotoxicity of Superstolides A and B against NSCLC make both molecules attractive synthetic targets. [48][49][50][51][52][53][54][55][59][60][61][62][63][64][65][66][67][68] Because of the scarcity of Superstolides A and B, extremely low isolation yield (0.003% for Superstolide A and 0.0003% for Superstolide B), and difficulty of sponge collection from 500-515 meters depth ocean, a highly efficient synthesis of these complex molecules (11 stereocenters, 6 double bonds, 3 rings, and 5 different functionalities) is required to supply sufficient quantities of the target material for future biological studies.…”

“…[72][73][74] Currently, chemotherapy and surgical resection are options for patients with NSCLC, of which chemotherapy shows only marginal benefits on survival and the quality of life still remains controversial. [75][76][77][78] Due to these reasons, the development of a novel generation of anti-NSCLC agents is highly desirable and needed.…”
