External Validation of the Patient-Reported Villalta Scale for the Diagnosis of Postthrombotic Syndrome

Abstract: Introduction:The Villalta scale is the endorsed tool to diagnose and grade the severity of post thrombotic syndrome (PTS); however, assessing presence and severity of PTS is time consuming and relies on both the clinician and patient's assessments. The patient reported Villalta scale version 2 (PRV2) is a visually assisted form that enables patients to self-assess presence and severity of PTS. Herein, we report on external validation of this tool. Methods: We assessed the agreement and kappa values of PRV2 to … Show more

“…Overall PTS score in the adult survivors was higher compared to the pediatric survivors (1 point/child, 1.6 points/adult). A VS ≥5 has been validated to distinguish clinical meaningful PTS in adults, 34,41 which explains why fewer adult survivors received a PTS diagnosis. Six pediatric survivors had collaterals (50%) (1 point), only three of these survivors had another symptom and ended up with two or three points.…”

“…Thus, the panel advocate for the Villalta scale to be used in conjunction with the three aforementioned diagnostic criteria. Ng et al (2022) recently validated the patient-reported Villalta scale (PRV) which may significantly reduce the burden on resources and clinical workload during patient followup [47]. The kappa value and agreement were 0.60 and 81%, respectively.…”

“…76 Early validation studies show good correlations for patient-reported Villalta and Villalta scores when patients are provided a visual aid to support self-assessment. 77,78 Many QOL self-assessment measures can be completed remotely. However, in contemporary research on behavioral and neurological conditions, mobile technology–supported ecological momentary assessments are routinely used to enable more frequent and more convenient data capture from study participants.…”
