Extraction Defect: Assessment, Classification and Management

Abstract: Tooth extraction is a traumatic procedure initiating a complex cascade of biochemical and histologic events that inevitably lead to a reduction of alveolar bone and soft tissue. These tissue alterations often lead to an esthetic compromise of the future implant restoration. The hard-and soft-tissue architecture surrounding the extraction defect largely dictates the course of dental implant treatment. The EDS or extraction defect sounding classification is a novel system introduced to simplify the decision-maki… Show more

“…Fracture of the buccal cortical plate may occur simultaneously with the removal of the root. 3 These factors can create bone defects with negative consequences, especially as they relate to immediate implant placement. In these clinical conditions, hand periotomes sometimes require hand mallets with dental assistants tapping on the handle to expand the PDL to separate the tooth from bone, and this can cause unnecessary discomfort for the patient as benign paroxysmal positional vertigo.…”

“…These may include sectioning of the roots or the use of a chisel and mallet to remove sufficient amounts of surrounding bone to allow extraction of the root; however, fracture of the buccal cortical plate may occur simultaneously with the removal of the root. 3 These factors can create bone defects that may have negative consequences, especially as they relate to subsequent or simultaneous implant placement. 4Y7 Several techniques have been proposed for minimally invasive tooth removal including an atraumatic method approach with a screw engaging the root, achieving more secure retention.…”

“…Evaluation of structural and functional state of bone was performed, identifying markers of bone tissue metabolism that react faster in comparison with densitometry. The most specific marker of bone formation is osteocalcin, which reflects the level of circulating bone formation rate [17]. For the quantitative analysis of serum osteocalcin level, the immune-ferment test Nordic Bioscience Diagnostics A/SN-MID Osteocalcin ELISA (Denmark) was used; referent indices range between 9.6-40.8 ng/ml for men, 8.4-33.9 ng/ml for women in premenopause, 9.5-48.3 ng/ml for women in post-menopause.…”

“…A fogak előrehaladott endodontalis eredetű gyulladás és jelentős szövethiány miatt kerültek eltávolításra. Az EDS (extraction defect sounding) -klasszifikáció az extrakciós alveolusokat négy csoportra osztja, és jól kiszámítható kezelési alternatívát ajánl [18]. Eseteink során egyforma, EDS 4-es defektusokat kezeltünk, amelyeknél jelentős volt a szöveti káro-sodás.…”
