COVID-19: Is There Evidence for the Use of Herbal Medicines as Adjuvant Symptomatic Therapy?

Background: Current recommendations for the self-management of SARS-Cov-2 disease (COVID-19) include self-isolation, rest, hydration, and the use of NSAID in case of high fever only. It is expected that many patients will add other symptomatic/adjuvant treatments, such as herbal medicines. Aims: To provide a benefits/risks assessment of selected herbal medicines traditionally indicated for "respiratory diseases" within the current frame of the COVID-19 pandemic as an adjuvant treatment. Method: The plant selection was primarily based on species listed by the WHO and EMA, but some other herbal remedies were considered due to their widespread use in respiratory conditions. Preclinical and clinical data on their efficacy and safety were collected from authoritative sources. The target population were adults with early and mild flu symptoms without underlying conditions. These were evaluated according to a modified PrOACT-URL method with paracetamol, ibuprofen, and codeine as reference drugs. The benefits/risks balance of the treatments was classified as positive, promising, negative, and unknown. Results: A total of 39 herbal medicines were identified as very likely to appeal to the COVID-19 patient. According to our method, the benefits/risks assessment of the herbal medicines was found to be positive in 5 cases (Althaea officinalis, Commiphora molmol, Glycyrrhiza glabra, Hedera helix, and Sambucus nigra), promising in 12 cases (Allium sativum, Andrographis paniculata, Echinacea angustifolia, Echinacea purpurea, Eucalyptus globulus essential oil, Justicia pectoralis, Magnolia officinalis, Mikania glomerata, Pelargonium sidoides, Pimpinella anisum, Salix sp, Zingiber officinale), and unknown for the rest. On the same grounds, only ibuprofen resulted promising, but we could not find compelling evidence to endorse the use of paracetamol and/or codeine. Plant essential oils are natural and aromatic compounds that are found in small amounts in plants and have many interesting applications as flavors and fragrances in perfumery, cosmetics, pharmaceutical, food, agriculture, industry and etc. Essential oils can be produced by various techniques including conventional, novel and biotechnology methods. can be considered as a good alternative to conventional methods due to short extraction time, higher efficiency and quality, non-degradable compounds due to heat, no pollution and etc. Recently, due to the limitations and disadvantages of methods of extraction from intact plants, scientists have been trying to move their way to biotechnology methods and make a variety of plant aromatic compounds by microorganisms. In the biotechnology method, there is no any concern about factors such climate conditions, supply shortages, natural disasters, plant diseases and etc. and a product with very high efficiency and quality is obtained.

Pomelo peel-derived essential oils have been gaining popularity due to greater demand for stress relief therapy or hair care therapy. In this study, we first performed optimization of parameters in the pomelo essential oil extraction process on a pilot scale to gain better insights for application in larger scale production. Then extraction kinetics, activation energy, thermodynamics, and essential oil quality during the extraction process were investigated during the steam distillation process. Three experimental conditions including material mass, steam flow rate, and extraction time were taken into consideration in response surface methodology (RSM) optimization. The optimal conditions were found as follows: sample weight of 422 g for one distillation batch, steam flow rate of 2.16 mL/min and extraction time of 106 min with the coefficient of determination R2 of 0.9812. The nonlinear kinetics demonstrated the compatibility of the kinetic model with simultaneous washing and unhindered diffusion with a washing rate constant of 0.1515 min−1 and a diffusion rate constant of 0.0236 min−1. The activation energy of the washing and diffusion process was 167.43 kJ.mol−1 and 96.25 kJ.mol−1, respectively. The thermodynamic value obtained at the ΔG° value was −35.02 kJ.mol−1. The quality of pomelo peel essential oil obtained by steam distillation was characterized by its high limonene content (96.996%), determined by GC-MS.
