Extradural haematoma: Trends in outcome over 35 years

Abstract: We have reviewed 35 years experience of extradural haemorrhage (EDH) in a large neurosurgical unit, based in two university hospitals, one dealing exclusively with children and the other a general hospital. A steady reduction in the mortality rate from 29 to 8.5% occurred during that period. A trend towards earlier diagnosis is noted and an increasing proportion of rural patients has been evident throughout the study period. During the time-period studied there were many significant developments: the establish… Show more

“…Patients with heterogenous clots had a poorer outcome [31] when compared with those with homogeneous clots. Presence of intradural associated lesions also influenced outcome [12,15,16,19,22,31,38]. Only two reports did not confirm this relationship [35,36].…”

“…There is a clear tread towards a better outcome in younger age groaps [I, 7, 8, i3, 20, 22, 23, 30, 31, 36], but statistical significance was shown only in two papers [16,38]. In two reports no significant relationship between age and outcome was found [19,35].…”

“…For this reason, the outcome of treatment of EDH has remained a yardstick to measure the efficacy of neurotrauma service 1 .The incidence of EDH among traumatic brain injury (TBI) patients has been reported to be in the range of 2.7 to 4% 2 . It has been estimated that EDH represents 2-3% of all head injuries in the pediatric population .…”
