Extranodal lymphoma arising within the maxillary alveolus: a systematic review

Abstract: ObjectivesExtranodal lymphomas affecting the head and neck infrequently arise within the jaw bones. Although clinical examination and conventional radiography may initially suffice for such lesions arising within the mandible, those arising within the maxillary alveolus generally require cross-sectional imaging because of the complex anatomy of this region. This study was performed to determine the prevalence, demographic characteristics, and clinical presentations of these lesions and the imaging modalities u… Show more

“…2 Malignancies arising within the alveolus may initially present as dental infections. 3 Although the clinical examination is of paramount importance, it can only penetrate as far as the eye can see, the fingers can palpate, and dental instruments can probe, or to the extent that percussion and vitality testing can assess. To examine deeper, particularly into the bones of the jaws, requires radiography.…”

“…The most frequent symptoms of NHL arising within the maxillary sinus are pain, swelling, nasal obstruction, paresthesia and numbness [25,30]. The patient had few or no typical complaints, suggesting the presence of periapical inflammation and maxillary sinusitis localized to the left upper premolar region; therefore, a definitive diagnosis was hardly determinable.…”

“…The presence of extra nodal lymphomas of the jawbones is exceptional [10,30]. Since very little data are available on the pathogenesis and clinical behaviour of HGBL, NOS lymphomas are referred to as "grey-zone" lymphomas, and there is no consensus on their standard therapy [23].…”

“…3,4,15 Most lymphomas involving the jaws are frequently associated with teeth, either causing tooth displacement, mobility or pain. 9,16 This presentation can lead to inappropriate dental treatment due to inflammatory conditions, especially when the lesion was located at the tooth root apex. Consequently, dental extractions, endodontic treatments, periodontal therapy and the use of different medications were some of the initial treatment options in these patients, with a resultant negative impact on patients.…”

“…Furthermore, any untimely delay in the recognition and diagnosis of these neoplasms may negatively impact the therapeutic management of affected patients. 9,10 Currently, the literature is scarce on describing the most common subtypes and radiological features of haematolymphoid malignancies affecting the OMF region. Therefore, this study aimed to analyse the clinicoradiological features of malignant haematolymphoid neoplasms affecting the jaws by assessing individual case reports available in the literature following a systematic review approach.…”
