Extratesticular Epidermoid Cyst with Extension into the Pelvis

Abstract: We report a case of simple epidermoid cyst of the scrotum with extension into the pelvis. Complete excision was performed transcrotally and the presence of epidermoid cyst confirmed by histopathological examination. Simple excision was sufficient for treatment of this very rare lesion. The patient is now in the follow-up period.

“…9 Three other case reports in the literature exhibited scrotal extratesticular epidermoid cysts extending in to the pelvis, but no attachments to the seminal vesicles were mentioned. 3,8 To our knowledge, this is the first case in the English literature. Extratesticular epidermoid cysts, if they are palpable, cannot be hidden as is the case with cysts in intratesticular locations.…”

“…Scrotal and extratesticular epidermoid cysts, as was the case in our patient, are very rare; the number of cases published in the English literature is less than 10. 1,3,[7][8][9] In our case scrotal epidermoid cyst was also attached to the anterior aspect of the seminal vesicles in the midline. There is only one reported case of a dermoid cyst arising from the posterior aspect of the prostate and seminal vesicles, but the cyst was in the pelvis masquerading as a full bladder.…”

“…8,9 Additional rare reports include patients with extratesticular epidermoid cysts and an epidermoid cyst of the penis, each with extension to the pelvis. 10,11 Epidermoid cysts of the buttock are rare. [12][13][14] Three such occurrences have been reported in male patients ages 39, 75, and 79 years, respectively.…”

“…[8][9][10][11] In 2 separately reported cases, pelvic masses were detected after the finding of extrates- ticular scrotal epidermoid cysts in 8-and 9-yearold boys, respectively. Similar to our case, the precise etiologies in these cases were unknown yet were considered to probably represent monolayer teratomas of germ cell origin.…”
