Facilitators and Barriers of Managing Patients with Multiple Chronic Conditions in the Community: A Qualitative Study

Abstract: Background About one-third of all adults worldwide were diagnosed with multiple chronic conditions (MCCs). Literature had found several challenges of providers and patients coping with managing MCCs in the community, urging the need for further study.
