Facteurs pronostiques de l’hématome cérébral spontané à la tomodensitométrie

Abstract

Objective: to determine the prognostic factors of cerebral heamatoma on ct-scan in patients with hemorrhagic stroke Materials and Methods: it was a prospective analytical study of 06 months duration, including 98 patients admitted in the Neurology ward of the Lomé campus university teaching hospital for ct-scan proved cerebral hematoma in hemorrhagic stroke (HS). The studied variables were age, sex, clinical signs, topography and volume of hematoma, mass-effet, mortality and hospitalisation duration. Results : 98 patients with cerebral hematoma were enrolled. The mean age was 53,04±10,9 years with 1.0 sex-ratio. HTA was the main risk factor (40.89%). Motor deficit (35.7%) was the most frequent clinical. The mortality rate was 41.8% (41 cases). Among dead patients, the hematoma was subcortical in 75.6%,with a volume greater than 30 cm3 in 634%, and 70.7% had a midline deviation of more than 10mm with a statiscally significant difference. The average duration of hospitalisation of dead patients or surviving patients had no significant relationship with the topography, the volume of the hematoma, or the brain midline deviation. Conclusion: The volume and topography, associated with brain midline displacement could be the prognostics factors for brain hematoma on ct-scan. RÉSUMÉ Objectif : déterminer à la tomodensitométrie (TDM) les facteurs pronostiques de l’hématome intracérébral de l’accident vasculaire hémorragique (AVCH). Matériels et méthodes : étude prospective analytique couvrant une période de 06 mois, portant sur 98 patients hospitalisés au service de Neurologie du CHU campus de Lomé, victimes d’un AVCH, avec un hématome intra parenchymateux confirmé au scanner cérébral. Les paramètres étudiés concernaient l’âge, le sexe, les signes cliniques, la topographie, le volume de l’hématome, les engagements, le pronostic vital et la durée du séjour hospitalier. Résultats : 98 cas d’AVCH avec un hématome intracérébral étaient colligés. L’âge moyen était de 53,04±10,9 ans et le sex-ratio de 1. L’HTA était le principal facteur de risque (40,89%). Les signes cliniques étaient dominés par le déficit moteur avec 35 cas (35,7%). 41 cas (41,8%) de décès étaient enregistrés. Parmi les patients décédés, l’hématome était sous cortical dans 75,6% des cas, le volume de l’hématome était supérieur à 30cm3 dans 63,4% des cas et dans 70,7% des cas, la déviation de la faux du cerveau était supérieure à 10mm avec une différence statistiquement significative. La durée moyenne d’hospitalisation des patients décédés ou survivants n’avait pas de relation significative avec la topographie, ni le volume de l’hématome, ni de l’engagement. Conclusion : Le volume et la topographie des lésions, associés à l’engagement seraient des facteurs du pronostic vital de l’hématome intracérébral à la TDM.
