INTRODUCTION Family planning is a right and a tool that offers the possibility of choosing how many children to have. Its importance lies in the possibility of avoiding an unwanted pregnancy and its consequences. Our objective was to determine the factors associated with discontinuing hormonal contraceptives in women of childbearing age who attended the La Libertad Health Center in January 2023. METHODS The study was observational, analytical, and cross-sectional. A total of 100 women of childbearing age who were users of hormonal contraceptives were included. Descriptive statistics were performed, frequency measurements and measures of central tendency were calculated, bivariate statistics were performed and the prevalence ratio (PR) was calculated, and a robust Poisson regression model was performed to assess the associated independent factors. All calculations were made with a confidence level of 95%. RESULTS The educational level (PR=1.74; 95% CI: 1.22-2.48, p=0.006), the distance to the health center (PR=7.32; 95% CI: 1.1-48.5, p=0.001), having presented adverse events (PR=26.38; 95% CI: 3.8-183, p=0.001), and that the health staff had not identified the need for contraception (PR=3.12; 95% CI: 0.87-11.10, p=0.01) were associated with stopping a hormonal contraceptive. After introducing the variables to the regression model, the only independently associated factor was having presented an adverse event with the use of hormonal contraceptives (adjusted prevalence ratio, APR=3.33; 95% CI: 2.1-5.2, p<0.001). CONCLUSIONS In this population, the factors associated with the discontinuation of hormonal contraceptives were education level, distance to the health center, having presented some adverse event with its use, and that health staff had not identified the need for contraception. The only independently associated factor was having presented an adverse event.

Objective: To identify the main reasons for early withdrawal of subdermal implant with etonogestrel after less than three years of use. Methods: Analytical cross-sectional study from January to December 2021, by stratified random sampling of 153 subdermal implant users of the Family Medicine Unit No. 80 in Morelia, Michoacán. The χ2 was used for frequency comparison; associations with sociodemographic data and main reasons for early withdrawal were reported, considering p<0.05 statistically significant. Results: The prevalent age group was 20-24 years with 75.8% (n= 116), the main reasons for early withdrawal of the subdermal implant were side effects 35.9% (n= 55), desire for pregnancy 19.6% (n= 30), medical recommendation 15.6% (n= 24), other reasons 15% (n= 23), performance of a definitive method such as bilateral tubal occlusion, and vasectomy by the partner 13.7% (n= 21). Subdermal implant withdrawal due to side effects, duration of use, marital status, and withdrawal due to desire for pregnancy were variables with statistically significant associations. Conclusions: The impact of side effects was the main reason for early withdrawal of the subdermal implant with etonogestrel.

Objetivo: identificar los principales motivos del retiro temprano del implante subdérmico con etonogestrel menor de tres años de uso. Métodos: estudio transversal analítico de enero a diciembre de 2021, mediante muestreo aleatorio estratificado de 153 pacientes usuarias de implante subdérmico de la Unidad de Medicina Familiar No. 80 en Morelia, Michoacán. Se utilizó χ2 para comparación de frecuencias; se reportaron asociaciones con los datos sociodemográficos y los principales motivos de retiro temprano, considerando p<0.05 estadísticamente significativa. Resultados: el grupo de edad prevalente fue de 20-24 años con 75.8% (n= 116), las principales causas del retiro temprano del implante subdérmico fueron por efectos secundarios 35.9 % (n =55), deseo de embarazo 19.6% (n= 30), recomendación médica 15.6% (n= 24), otros motivos 15% (n= 23), la realización de un método definitivo como oclusión tubárica bilateral y vasectomía por parte de la pareja 13.7% (n= 21). El retiro del implante subdérmico por efectos secundarios, el tiempo de uso, el estado civil, así como el retiro por deseo de embarazo fueron variables con asociaciones estadísticamente significativas. Conclusiones: la repercusión de los efectos secundarios fue el principal motivo de retiro temprano del implante subdérmico con etonogestrel.
