Factors Associated With Medication‐Overuse Headache in Patients Seeking Treatment for Primary Headache

Abstract: Items that assess headache-related disability, use of combination medications, and fear of pain help identify patients who are currently overusing acute headache medications and may serve as indicators of treatment progress. Future studies should apply similar analytic approaches longitudinally to identify headache sufferers at risk for medication-overuse headache prior to headache progression.

“…Medication overuse contributes to the development of chronic migraine and MOH . Psychological factors include depression and anxiety, fear of the next migraine attack, behavioral factors, and a psychological dependence component in some patients . In the present study, we tested the hypothesis that high achievement motivation predisposes migraine patients to increased use of acute headache medication, and/or MOH, because highly achievement motivated patients might fear to perform poorly at work or during daily life because of their headache.…”

“…Psychological factors like anxiety and depression, and pain‐related behaviors also play a crucial role . Migraine and/or MOH patients often exhibit a high anticipatory anxiety with respect to the next attack . The attempt to “solve pain” by use of acute headache medication is more common in migraine patients with than without MOH .…”

“…28 Headache patients with signs of medication-overuse often report high disability level, depressive and anxious symptoms, fear of pain and frequency of headache episodes. 70 Using therapeutic exercise as an approach to reduce medication use could be a way to reduce headache impact.…”

“…Analgesic consumption must be carefully evaluated. Frequent use of all types of pain-relieving drugs, in particular triptans, ergotamine, opioids, caffeine, meprobamate and codeine-containing products, may be associated with rebound headache, which itself is associated with greater pain-related disability 53,54. Withdrawal of treatment often leads to improvement and evolution to episodic headache.…”
