Factors Influencing the Outcome of Regenerative Therapy in Mandibular Class II Furcations: Part I

Abstract: The successful clinical closure of Class II furcations was achievable at 1 year following combination therapy with an ePTFE membrane and DFDBA. The highest frequency of clinical furcation closure was observed in early Class II defects. Furcations with vertical or horizontal bone loss of 5 mm or greater responded with the lowest frequency of complete clinical closure. Nevertheless, complete furcation closure was achievable in 50% of molars with extensive bone loss. Also, 15 out of 22 (68%) of all residual defec… Show more

“…51 A similar scenario is observed in the treatment of furcation regions 52 and after regenerative procedures. 53 Recent studies have suggested that the adjunct use of local and systemic antimicrobial therapy may improve the clinical results obtained with scaling/ root planing 54 and guided tissue regeneration in smokers. 55 Machion et al 56,57 showed that the association of scaling and root planing to local 10% doxycycline in the treatment of smokers with chronic periodontitis may lead to better clinical results than mechanical therapy alone.…”

“…The patient-related factors that have a negative influence on the regeneration of these lesions include smoking, stress, diabetes mellitus, acquired immunodeficiency syndrome and other acute and debilitating diseases, and the presence of multiple deep periodontal pockets. Age, gender and type of periodontal disease do not seem to play a major role in regenerative therapy (10,16).…”

“…The predictability of GTR in class II furcations is also strongly influenced by local factors, such as furcal anatomy, defect morphology, thickness of gingival tissue, and tooth mobility (8,10,(26)(27)(28)(29).…”

“…The demonstration of these properties in vitro has led to the speculation that such factors play an important role in soft and hard tissue regeneration. Recombinant human platelet-derived growth factor-BB (PDGF-BB) is a polypeptide growth factor shown by preclinical and clinical studies to be effective in the induction of bone regeneration and soft tissue wound healing 2,3) . PDGF-BB has plasma alpha particles.…”
