Factors Influencing the Results of Double-Valve Surgery in Patients with Fulminant Endocarditis: The Importance of Valve Selection

Abstract: The mortality in patients with destructive endocarditis requiring DVS depends mostly on the patients' preoperative hemodynamic situation. The risk of reinfection can be minimized if valve substitutes are properly selected (homografts, Shelhigh No-React SuperStentless and No-React BioConduit in the aortic position, or Shelhigh BioMitral in the mitral position). Concomitant mitral valve reconstruction procedures do not increase the risk of mitral reinfection.

“…6 Indeed, preliminary results in 2003 and 2004 showed that No-React valves resist infection as well as homografts and they were found to have good hemodynamic characteristics. 7,8 In 2008, we reported our results with a larger cohort of 255 patients and the early and midterm results confirmed our expectations. 9 In this study, we have reviewed 402 AIE patients in whom No-React bioprosthesis was implanted between February 2000 and February 2011.…”

“…In valve replacement, the choice of valvular substitute is debated [37]. Beyond biologic and mechanical prostheses, homograft is also an alternative, and is preferably used in periannular abscess, which is often the case in multivalvular IE.…”

“…Further studies using No-React ® tissue as a patch for the Norwood procedure caused no anti-HLA antibodies whereas allografts induced the production of antibodies [ 13 ]. Indeed, clinical studies with No-React ® valves, receiving an identical treatment to the No-React ® BioConduit, have shown a high resistance to infection [ 14 , 15 ]. However, a recent report has warned on the possibility that this valved conduit may undergo degeneration [ 16 ].…”
