In this retrospective data analysis, it was examined whether the mode of anesthesia influenced postoperative haemorrhage in patients with first time complete silicone oil removal in 2002, 2005, 2010, 2014 - 2016 in our clinic and if there was a change in frequency over time. Haemorrhage was defined as a postoperative haemorrhage within 2 weeks after the operation and which led to revision vitrectomy. In all, there were 939 operations and 9 cases of postoperative surgery requiring revision in the analysed years. Even though the number of patients with intubation anesthesia (355) was smaller than the number with local anesthesia (584), the majority of patients with postoperative haemorrhage had been given intubation anesthesia (6). This difference corresponds to a trend (p = 0.089) towards a lower rate of postoperative haemorrhage with local anesthesia, which could encourage the surgeon to perform silicone oil removal under local anaesthesia. Over the course of time, there was a decline in postoperative haemorrhage. In 2002, 2005 and 2010 there were 7 cases of postoperative haemorrhage, but only 2 in the years 2014 - 2016. Aside from the higher proportion of operations under local anaesthesia, the decline in the frequency of postoperative haemorrhage may be attributed to "smoother extubation", the emergence of trocar-guided surgical procedure and the use of anti-VEGF therapy - all leading to a change in the spectrum of illnesses treated by silicone oil instillation.

Visual Outcome and Post Operative Complications After Silicone Oil Removal in Pseudophakic Vitrectomized Patients

A total of 32 eyes of 32 patients were enrolled in the study after fulfilling inclusion and exclusion criteria using nonrandom consecutive sampling technique. All patients had pars plana vitrectomy with silicone oil done 6 months ago and were pseudophakic. Silicone oil removal was carried out in all patients and visual outcome and surgical complications assessed on 1st and 14th post-operative day. Final examination was done after six months. Statistical analysis was done by using Statistical Package for Social Sciences (version 21) by applying paired sample t-test. RESULTS:Amongst 32 patients, 20 (62.5%) were male and 12 (37.5%) were female. Age of the patients ranged from 16 to 60 years with a mean age of 35±13.97 years. Pre operatively mean best corrected visual acuity (BCVA) was 1.45±0.52 Log Mar. On the last post-operative follow up after six months mean BCVA was 1.21±0.55 Log Mar. Visual acuity improved in 24 (75%), remained stable in 3 (9.4%) and worsening in visual acuity was seen in five (15.6%) cases. Visual improvement was statistically significant (p-value 0.001) using paired ttest. Most common complications were retinal detachment (n=4: 12.5%), secondary glaucoma (n=4: 12.5%) and epi-retinal membrane (n=2; 6.3%). CONCLUSION:Vision improves in majority of pseudophakic patients after silicone oil removal.

Patients with rhegmatogenous retinal detachment (RRD) require face-down positioning (FDP) for 3–6 months or longer after pars plana vitrectomy (PPV) combined with silicone oil (SO) tamponade. This paper aimed to identify the factors that influenced FDP compliance. This study adopted semi-structured interviews with patients who require FDP after SO tamponade. Constructivist grounded theory was utilized in this study. The qualitative data was analyzed and coded via NVivo 11.0 through open coding, axial coding and selective coding. Twenty-four RRD patients were involved. The interviews yielded five main themes that defined home FDP compliance were identified: posture discomfort, doctor-patient communication, psychological factors, occupational character, and family factors. A theoretical model of the influencing factors of postural compliance of FDP was constructed based on the interview analysis. A variety of factors can affect FDP conformity. We can increase compliance of RRD patients by enhancing comfort, encouraging doctor-patient communication, providing comprehensive care, promoting community-based intervention, and strengthening family education.
