Factors Predicting COVID-19 Vaccination Uptake Among Men Who Have Sex With Men in China: An Observational Prospective Cohort Study

Abstract: BackgroundThe coronavirus disease 2019 (COVID-19) pandemic has disproportionately affected sexual minorities including men who have sex with men (MSM). This study investigated factors associated with the uptake of COVID-19 vaccination among MSM in China.MethodsInclusion criteria were: (1) born biologically male, (2) had oral or anal sex with men in the past year, (3) aged at least 18 years, and (4) lived in Shenzhen, China. “…Assuming the participation rate of valid households to be 55–60%, we needed to screen about 4000 households in order to recruit 400 eligible participants. The same approach for sample size planning was used in published studies [ 34 , 35 , 36 , 37 ].…”

“…Sexualized drug use was defined as the use of any of the following psychoactive substances [i.e., ketamine, methamphetamine, cocaine, cannabis, ecstasy, Dormicum/Halcion/Erimin 5/non-prescription hypnotic drugs, heroin, codeine from cough suppressants, amyl nitrates (poppers), γ-hydroxybutyrate (G water), and Foxy] before or during sexual intercourse ( 19 , 26 ). The measurements of these sexual risk behaviors were also adapted from previous studies targeting Chinese MSM ( 27 29 ). Four variables were created by comparing the presence of such behaviors at follow-up vs. baseline (response categories: 1 = no such behavior either at baseline or follow-up, 2 = with such behavior at baseline only, 3 = with such behavior both at baseline and follow-up, and 4 = with such behavior at follow-up only).…”

“…[18][19][20] Moreover, one study showed that baseline COVID-19 preventative behaviors during sex among MSM in China, such as asking partners about their symptoms, washing their hands before and after sex, and sanitizing before and after sex, were associated with COVID-19 vaccine receipt. 21 The co-occurrence of behaviors presents an important opportunity for multipronged interventions, which aim to reduce several risk behaviors concurrently. [22][23][24] There is also evidence to suggest that the number of risk behaviors in which youth engage increases with age, pointing to the need for early intervention.…”

“…Other studies have shown that the perception of risk taking may have shifted because of the pandemic, where individuals perceive a heightened sense of risk, 17 and researchers have documented changes in specific behaviors undertaken, such as the increased adoptions of sexting, virtual sex, and pornography use or a rise in the co-occurrence of drug use and sex during the pandemic 18–20 . Moreover, one study showed that baseline COVID-19 preventative behaviors during sex among MSM in China, such as asking partners about their symptoms, washing their hands before and after sex, and sanitizing before and after sex, were associated with COVID-19 vaccine receipt 21 …”
