PurposeThe incidence of retinal detachment in Blacks is generally considered to be low though there are few supporting studies in Africa. This study, thus, aimed at describing the clinical profile of patients with retinal detachment in Southwest Ethiopia.MethodsA hospital-based study was done on all consecutive retinal detachment patients who presented to Jimma University Hospital over six months period. A semi-structured questionnaire was used to collect patients’ sociodemographic characteristics and clinical history. Comprehensive anterior and posterior segment eye examinations were done and risk factors were sought for. Statistical tests were considered significant if P < 0.05.ResultsA total of 94 eyes of 80 patients (1.5%) had retinal detachment (RD) and about 69% of patients were symptomatic for over a month before presentation. The mean age was 41.4 years (SD ±16.5). Fourteen patients (17.5%) had bilateral RD. At presentation, 61 eyes (64.9%) were blind from RD and 11 (13.8%) patients were bilaterally blind from RD. Rhegmatogenous RD was seen in 55 eyes (58.5%) and tractional RD in 22 eyes (23.4%). The most common risk factors were ocular trauma (32 eyes, 34.0%), myopia (23 eyes, 24.5%), posterior uveitis (13 eyes, 13.8%) and diabetic retinopathy (9 eyes, 9.6%). Most retinal breaks (25 eyes, 43.1%) were superotemporal and horse-shoe tear was the most common (19 eyes, 20.2%). Macula was off in 77 eyes (81.9%) and 38 eyes (69.1% of RRD eyes) had grade C proliferative vitreoretinopathy (PVR). Macular status was significantly associated with PVR (P=0.011), and duration of symptoms (RR=1.25, 95%CI: 1.059-1.475, P=0.040).ConclusionsA significant numbers of patients with ocular problem had retinal detachment, and nearly two third of the patients presented late. Trauma and myopia were the most important risk factors. People should be educated to improve their health seeking behavior and use eye safety precautions to prevent ocular trauma.

ObjectiveDue to increment in volume of cataract surgery and diabetic retinopathy, the incidence of retinal detachment (RD) is expected to rise in the future. However, little is known about the clinical patterns and risk factor of RD in Ethiopia. This study was aimed to determine the clinical pattern and risk factors of RD at University of Gondar Tertiary eye care and training center (UOG-TECTC).MethodsA hospital based prospective cross sectional study was done involving all consecutive patients with retinal detachment seen over six months period at the UOG-TECTC. Structured questionnaire was used to extract the required data which included demography, ophthalmic history and clinical examination results. Data were entered into SPSS version 20 and analyzed.ResultsA total of 73 eyes of 65 patients with RD were analyzed. Two-third of patients were males 42(64.6%) and the median age was 49 years. Rhegmatogenous RD (RRD) was the commonest type of RD found in 58(89.2%) patients. Three quarter of the patients, 49 (75 %), came after 1 month of the symptom occurrence; out of which 20 (41%) visited the center after 1year duration. The commonest identified risk factors were trauma 19(29%) followed by pseudophakia 11(17%). More than three-quarter of the patients 53(81.5%) had detached macula and 60 (92.3%) of the involved eyes were blind at presentation.Conclusion and recommendationsRRD was the commonest type of RD seen in our patients. Majority of the patients visited our center late after macular detachment and trauma was the commonest risk factor. People should be educated to seek health care immediately after the onset of symptoms. Ocular safety precautions should be advised to decrease the risk of ocular trauma. The center should be equipped to manage patients with RD.

Neglected cause of retinal detachment: a hospital-based case-control study on occupational heavy lifting as a risk factor

Background Heavy lifting may lead to sudden increase in venous, intra-abdominal, and intraocular pressure which in turn may cause retinal detachment (RD). The epidemiological evidence for this association is still inconclusive. This study was carried out to investigate the relationship between occupational heavy lifting and RD. Methods A case-control study was carried out on 151 RD cases and 113 controls free of RD attending the ophthalmology outpatient clinic at Ain Shams University. Personal, medical, and occupational data were collected using interview questionnaires in addition to conducting full ophthalmologic examination. Results The mean age of study participants was 45.8 ± 9.1 years (46.8 ± 8.9, 44.4 ± 9.2 for RD cases and controls respectively). Statistically significant differences were found between cases and controls regarding years of working, occupational categories, frequency of occupational heavy lifting, non-work heavy lifting, history of head trauma, history of eye surgeries, and family history of RD. Multivariate logistic regression analysis showed that lifting (Odds ratio (OR) = 4.8, p < 0.0001), history of head trauma (OR = 3.3, p = 0.013), diabetes mellitus (DM) (OR = 4.96, p < 0.0001), and previous eye surgeries (OR = 3.5, p = 0.003) increased the risk of RD. Conclusion Occupational heavy lifting is associated with RD. Occupational categories, duration of lifting heavy objects during work and family history of RD had a significant effect on RD. An ergonomic approach should be adopted and practiced as it has a significant impact on reducing the risks of lifting and carrying heavy objects. The workplace’s design (including having appropriate mechanical aids available) is also of significant importance to reducing the risks. During the patient’s visit, ophthalmologists should consider and look for the occupational heavy lifting history as a potential risk factor of the patient’s symptoms.
