Factors related to lymph node harvest: does a recovery of more than 12 improve the outcome of colorectal cancer?

Abstract: Number of LN metastases, lymphocyte response, type of surgical resection, age of patient and colorectal surgeon can predict the LN harvest. Survival in colorectal cancer, however, is probably more influenced by the performance of the operation by an expert surgeon than by recovery of more than 12 LNs.

“…Furthermore, T-classification and higher tumor stage were significantly associated with higher lymph node count. Both associations are in the line with literature data showing associations of lymph node retrieval with T-classification [13, 21, 23, 32] and AJCC/UICC stage [11, 13, 14, 21, 3235]. Also, tumor size is, according to previous studies, an established predictor of lymph node yield [11, 13, 23, 29, 31, 36] that could be confirmed by our study.…”

“…Also, since several studies failed to prove prognostic impact [1315, 34, 36, 40]. These negative results may be due to different methodologies and heterogeneous study populations including different stages, both colon and rectum cancers, different cut-off values, but also small sample sizes.…”

“…Adequate resection of LNs in surgery of CRC is an important factor in tumor staging. Several factors have been shown to have a positive correlation with the number of LNs harvested, such as patient age <60 years, right‐sided tumor location, advanced tumor stage, longer length of resected bowel segment, and expertise of the pathologist and surgeon . In this study, the median numbers of LNs harvested were 26 in the laparoscopic and 20 in the open group.…”

“…The histopathological variables assessed, defined in terms of undesirable outcomes, were traumatically disrupted perirectal fascial envelope as determined macroscopically by the surgeon from the fresh, unopened operative specimen; tumor at or within 1 mm of the circumferential line of resection; a lymph node yield less than 12; and a distal margin of less than 1 cm …”
