Failure to Improve Parameters of Lactose Maldigestion using the Multiprobiotic Product VSL3 in Lactose Maldigesters: A Pilot Study

Abstract: Lactose maldigestion is a common genetic trait in up to 70% of the world's population. In these subjects, the ingestion of lactose may lead to prebiotic effects which can be confirmed by measurement of breath hydrogen. After a period of continuous lactose ingestion, colonic bacterial adaptation is measurable as improved parameters of lactose digestion. There may be inherent benefits in this process of adaptation which may protect against some diseases. We attempt to link therapeutically beneficial probiotics (… Show more

“…The contrary situation also occurs. In a previous study in lactose malabsorbers, clinical adaptive characteristics could not be reproduced by feeding the multispecies probiotic VSL3 [52]. A similar outcome with other probiotics was reported in two other studies [53,54].…”

“…Beneficial effects can be caused by microbial β‐galactosidase, a delayed GI transit time, a reduced sensitivity to symptoms and positive effects on intestinal functions and the colonic microbiota 125 . An improvement in parameters for lactose maldigestion has been found by some authors, 126–128 but not by others 129–131 . Recently Levri et al.…”

“…125 An improvement in parameters for lactose maldigestion has been found by some authors, [126][127][128] but not by others. [129][130][131] Recently Levri et al 132 reviewed controlled studies only and concluded that probiotic supplementation in general did not alleviate symptoms and signs of lactose intolerance but some evidence was found that specific strains might be effective.…”

“…The sample size (48 patients) and study design (randomized, placebo-controlled, single-blind) are important strengths of the study, as most of the previous studies assessing the effect of probiotics on lactose intolerance enrolled fewer patients or were nonrandomized. 11,20,[24][25][26][27] Statistical significance was robustly validated in ITT and PP populations, as well as after adjusting for baseline imbalances as recommended by the EMA guideline. 19 The study was powered to detect an effect on the total symptom score.…”
