Faktor-faktor yang berhubungan dengan Multidrug Resistant Tuberculosis (MDR-TB) pada pasien tuberkulosis paru

Abstract

Background: Anti-TB drug resistance is basically a man-made phenomenon as a result of inadequate treatment of TB patients. Based on the data in the Sub-Division of Medical Records, Dr. H. Abdul Moeloek Hospital Lampung Province recorded as many as 62 MDR-TB patients in the period 2022.Purpose: To determine factors associated with Multidrug Resistant Tuberculosis (MDR-TB) among patients with pulmonary tuberculosis (TB)Method: A quantitative with a case control design. The study population was all patients from the MDR Polyclinic, Regional General Hospital Dr. H. Abdul Moeloek Lampung Province in February 2021 a total of 62 people. The sample is 62 cases and 62 controls. The statistical test used is the chi square test with the help of a computer.Results: Showed that, out of 124 respondents, the majority of respondents were in the productive age range of 76 respondents (61.3%), high socioeconomic status of 74 respondents (59.7%), abnormal nutritional status of 82 respondents (66.1%) , adherence to taking medication was low by 68 respondents (54.8%), comorbidities by 79 respondents (63.7%), smoking by 72 respondents (58.1%).Conclusion: There was no relationship between age with MDR TB cases, but there was a relationship between socioeconomic, nutritional status, medication adherence and smoking habits with MDR TB cases and to prevent an increase number of MDR-TB in Lampung Province. Suggestion: By using a predictive index of MDR-TB incidence both at the community level and at the health worker level and the community level it is useful for initial screening for MDR-TB prevention in TB patients.Keywords: Multidrug Resistant Tuberculosis (MDR-TB); Patient; Pulmonary tuberculosisPendahuluan: Resistensi obat anti TB (OAT) pada dasarnya adalah suatu fenomena buatan manusia sebagai akibat dari pengobatan pasien TB yang tidak adekuat. Berdasarkan data di sub-bagian rekam medis RSUD Dr. H. Abdul Moeloek Provinsi Lampung tercatat sebanyak 62 pasien MDR-TB pada periode tahun 2022.Tujuan: Mengetahui faktor-faktor yang berhubungan dengan Multidrug Resistant Tuberculosis (MDR-TB) pada pasien Tuberkulosis paruMetode: Penelitian kuantitatif dengan rancangan case control. Sampel sejumlah 62 orang. Sampel 62 kasus dan 62 kontrol. Uji statistik yang digunakan adalah uji chi square dengan bantuan komputer.Hasil: Dari 124 responden didapatkan hasil bahwa sebagian besar responden dalam rentang usia produktif sebanyak 76 responden (61,3%), sosial ekonomi tinggi sebanyak 74 responden (59,7%), status gizi tidak normal sebanyak 82 responden (66,1%), kepatuhan minum obat rendah sebanyak 68 responden (54,8%), penyakit penyerta sebanyak 79 responden (63,7%), merokok sebanyak 72 responden (58,1%).Simpulan: Menunjukan tidak terdapat hubungan usia dengan kasus TB MDR namun ada hubungan sosial ekonomi, status gizi, kepatuhan minum obat dan kebiasaan merokok dengan kasus TB MDR.Saran: Perlunya melakukan pencegahan terjadinya peningkatan TB-MDR di Provinsi Lampung dengan menggunakan indeks prediktif kejadian TB-MDR baik di tingkat masyarakat maupun petugas kesehatan. Pada tingkat masyarakat berguna untuk untuk skrining awal untuk pencegahan TB-MDR pada penderita TB.
