Family History and Serology Predict Crohn's Disease after Ileal Pouch-Anal Anastomosis for Ulcerative Colitis

Abstract: Purpose-About 5 to 10 percent of patients undergoing ileal pouch-anal anastomosis with a diagnosis of ulcerative colitis are subsequently diagnosed with Crohn's disease. Preoperative predictors for Crohn's disease post-ileal pouch anal-anastomosis have not been prospectively defined.Methods-Two hundred thirty-eight consecutive patients with ulcerative colitis or indeterminate colitis undergoing ileal pouch anal-anastomosis were prospectively enrolled into a longitudinal database. Clinical factors were assessed… Show more

“…Only rarely does refractory or recurrent pouchitis occur because of the missed diagnosis of CD ( 341 ), which may occur more commonly in patients with a family history of CD or preoperative antisaccharomyces cerevisiae antibody seropositivity ( 342 ). CD of the pouch should be suspected if a de novo fi stula develops 6 -12 months aft er ileostomy takedown in the absence of postoperative leak, abscess, or sepsis.…”

“…pANCA has been shown to be associated with less severe disease, UC-like disease and to be negatively associated with small bowel complication [36,39] . ASCA positivity has been associated with CD of the pouch after ileal pouch-anal anastomosis (IPAA) [40] .…”

“…In addition to a preoperative diagnosis of CD, 12 other reported risk factors for the development of CD of the pouch include sero-positive anti-Saccharomyces cerevisiae-IgA, 25 a family history of CD, 25,26 being active smoker, 27 a long duration of pouch, and a preoperative diagnosis of IC. 27,28 Different phenotypes of CD of the pouch were shown to be associated with different risk factors.…”
