Far Advanced Malignant External Otitis: Report of a Survival

Abstract: Malignant external otitis has been the subject of several reports since it was described by Chandler in 1968. Cranial nerve involvement has been recognized as an ominous prognostic sign. Mortality has been about 50 percent (10/19) with facial nerve paralysis, and over 80 percent (9/11) with multiple cranial nerves involved.

“…Progression of the infection has been associated with multiple cranial nerve neuropathies, predominantly facial nerve neuropathy. [1][2][3][4] The inflammatory process apparently interferes with the conductive abilities of the facial nerve, 5,6 and in severe cases, the integrity of the nerve itself may be completely disrupted, with replacement by granulation tissue. Therefore, facial nerve decompression will not restore normal facial nerve function.…”

“…Facial nerve involvement is reportedly associated with a mortality rate of up to 50%. [1][2][3][4][5] In 1985, Corey et al 7 reviewed the findings of 83 patients with MEO reported in the literature up to that time. Fifty-eight had facial paralysis (70%).…”

“…The initial literature search yielded 104 abstracts warranting further appraisal. Overall, 58 papers were included in the analysis 2 , 6 , 7 , 9 13 , 15 , 16 , 20 67 . Inclusion was considered if papers contained sufficient case data within the published article concerning age, sex, presenting features, further symptoms, clinical signs or investigations, and outcome of suspected cases.…”
