Fatal Abdominal Compartment Syndrome Due to Constipation: A Case Report

Abstract: Introduction: Abdominal compartment syndrome (ACS) is a rare condition in which increased intra-abdominal pressure causes multiorgan dysfunction through decreased perfusion. Causes of this condition are variable, and early recognition is critical for favorable patient outcomes. Measurement of bladder pressure is recommended for diagnosis. Case Report: A 64-year-old female on clozapine with a two-year history of chronic constipation presented to the emergency department in extremis with a protuberant abdomen. A… Show more

“…With the disruption of the intestinal barrier, bacterial translocation could lead to the initiation of systemic inflammatory response syndrome, multiple organ dysfunction syndromes, or even death [Puleo 2011]. Defecation delay also may result in increased intra-abdominal pressure, which can further lead to dysfunction of the abdominal organ [Al Ali 2021; Kongkatong 2022].…”

“…In the adult population, there is very limited literature on ACS related to constipation in adults. A case report by Kongkatong et al [ 9 ] describes an adult patient with chronic constipation on long-term clozapine with a likely diagnosis of ACS. Despite undergoing urgent decompressive laparotomy, the patient suffered a cardiac arrest on table and subsequently demised.…”

“…Other complications of chronic constipation include abdominal compartment syndrome (ACS), defined as organ dysfunction caused by an increase in intra-abdominal pressure greater than 20 millimeters of mercury. The World Society of the Abdominal Compartment Syndrome (WSACS) classifies this condition by its underlying cause: decreased abdominal compliance (e.g., burns); increased intra-abdominal contents (e.g., hemoperitoneum, ascites); increased intraluminal contents (e.g., intestinal volvulus, ileus, and constipation), capillary leak/fluid resuscitation, and miscellaneous causes such as obesity and peritonitis [16].…”
