Fatal Knee Dislocation in a Morbidly Obese Jehovah’s Witness. A Case Report

Abstract: We describe a case of a morbidly obese Jehovah's Witness who sustained a popliteal artery and vein transection after a spontaneous knee dislocation. Following surgical repair, the patient fatally deteriorated, while blood products had to be withheld according to the patient's preoperative request.

“…Recently, cases of obese patients sustaining knee dislocations from seemingly innocuous lowvelocity mechanisms have been reported. 1,7,8,13,30,41 Given the limb-threatening nature of these injuries and the challenges in diagnosis inherent to this particular subpopulation, ultra-low velocity (ULV) knee dislocations warrant further investigation to characterize the epidemiology, risk factors, and results of operative and nonoperative management. Complete knee dislocation is estimated to account for only 0.001% to 0.013% of all orthopaedic injuries, making it a relatively rare injury.…”

“…Using these methods, survival has been reported following high volume blood loss in JW patients including ruptured aortic dissection ( 3 ), splenic laceration ( 4 ), poly-trauma ( 5 , 6 ) and subtotal colectomy for bleeding ( 7 ). However, death following surgical interventions for traumatic injuries in JW patients has also been reported ( 8 , 9 ).…”
