Background Methanol poisoning (MP) occurs often via ingestion, inhalation, or dermal exposure to formulations containing methanol in base. Clinical manifestations of MP include gastrointestinal symptoms, central nervous system (CNS) suppression, and decompensated metabolic acidosis occurred with blurred vision and early or late blindness. Objective This study reviewed the clinical manifestations, laboratory and radiology findings, and treatment approaches in MP. Discussion Methanol is usually rapidly absorbed after ingestion and metabolized by alcohol dehydrogenase (ADH), then distributed to the body water to reach a volume distribution approximately equal to 0.77 L/kg. It is also eliminated from the body as unchanged parent compounds. Clinical manifestations of MP alone initiate within 0.5–4 h after ingestion and include gastrointestinal symptoms and CNS suppression. After a latent period of 6–24 h, depending on the absorbed dose, decompensated metabolic acidosis occurs with blurred vision and early or late blindness. Blurred vision with normal consciousness is a strong suspicious sign of an MP. The mortality and severity of intoxication are well associated with the severity of CNS depression, hyperglycemia, and metabolic acidosis, but not with serum methanol concentration. After initial resuscitation, the most important therapeutic action for patients with known or suspected MP is correction of acidosis, inhibition of ADH, and hemodialysis. Conclusion Since MP is associated with high morbidity and mortality, it should be considered seriously and instantly managed. Delay in treatment may cause complications, permanent damage, and even death.

Background: Asphalt is a mixture of aliphatic and aromatic hydrocarbons. It also contains different kinds of heavy metals. Hot mix asphalt emits hazardous volatile substances. Asphalt is used in many industries, causing serious adverse health effects for workers. Therefore, investigation of the health status of workers is of crucial importance. The present study aimed to assess the impact of asphalt vapor on hepatic and renal function tests parameters and the clinical parameters in road paving workers compared to healthy subjects. Methods: In a cross-sectional study, 41 road asphalt paving workers and 120 healthy non-exposed subjects were selected. The serum enzymes of aspartate aminotransferase (AST), alanine aminotransferase (ALT), alkaline phosphatase (ALP), total bilirubin, triglyceride, urea, and creatinine levels were assessed using diagnostic kits. A medical practitioner examined all subjects for any possible medical condition. Results: Liver and kidney function test parameters were significantly higher in road asphalt paving workers compared to their healthy counterparts (P<0.05 for all parameters). About 63% of workers exposed to asphalt fumes for more than 10 years showed cardiac, pulmonary, and metabolic complications. Conclusion: Significant elevations in liver and kidney function test parameters suggest that exposure to asphalt fumes is associated with hepatic and renal dysfunction in asphalt-exposed workers.

Background: Nowadays, erythropoietin is employed as a substitute therapy for eye ailments, particularly optic nerve damage induced by methanol toxicity. Despite its benefits, erythropoietin can have negative effects on blood factors. These effects may include thrombosis, stroke, seizures, and changes in hematological function. This study examined the adverse effects of erythropoietin on individuals suffering from optic neuropathy caused by methanol. Methods: This study was a prospective descriptive study at Loghman Hakim Hospital, Tehran City, Iran, in 2022. The study involved individuals who suffered from methanol poisoning and were administered erythropoietin twice a day for three days, with a dosage of 10000 units each time. Patients were examined up to 6 weeks after discharge in terms of increased hemoglobin levels, seizures, central vascular thrombosis, and heart attack or stroke. The data was analyzed by SPSS software, version 18. Results: The study comprised of 37 patients. No patient reported seizures, heart and cerebral strokes, and deep vein thrombosis except polycythemia. After being discharged from the hospital, the mean hemoglobin levels increased by 2.01% six weeks later compared to the levels measured during admission. Moreover, there was a 2.4% rise in hemoglobin levels six weeks after discharge when compared to the levels measured two weeks after being discharged. Conclusion: Using erythropoietin to treat optic neuropathy can improve vision, but it also carries the risk of increasing hemoglobin levels, which must be monitored closely in treated patients.
