The acute effects of thoracentesis on arterial oxygenation were observed in 19 studies on 17 patients. Arterial blood samples were obtained prior to, immediately after, and 1 h after thoracentesis. Unpredictable changes in the arterial oxygen tension, ranging from – 22 to + 19 mm Hg were found immediately following thoracentesis. It was concluded that significant hypoxemia may result from thoracentesis due to aberration of ventilation-perfusion relationships. It is further suggested that oxygen therapy be routinely administered during and immediately after the procedure

ResumoO edema pulmonar de re-expansão é uma complicação pouco frequente que pode surgir após tratamento de colapso pulmonar prolongado secundário a derrame pleural, pneumotórax e atelectasia. A sua apresentação clínica é variável podendo manifestar-se apenas por sintomas ligeiros, contudo, cursa com insu ciência respiratória nos casos de maior gravidade. Apesar de se tratar de uma entidade infrequente apresenta mortalidade elevada. O tratamento é sintomático e assenta essencialmente em medidas de suporte hemodinâmico e ventilatório. Os autores descrevem o caso de uma doente de 37 anos que desenvolve um quadro fulminante de edema pulmonar de re-expansão bilateral acompanhado por insu ciência respiratória e choque hipovolémico após colocação de drenagem torácica para descompressão de pneumotórax espontâneo à direita.Palavras-chave: Edema pulmonar de re-expansão, pneumotórax, choque hipovolémico, insu ciência respiratória. AbstractReexpansion pulmonary edema is an uncommon complication that can arise after treatment of prolonged pulmonary collapse secondary to pleural effusion, pneumothorax and atelectasis. Clinical presentation is variable and can be manifested only by mild symptoms; however, it evolves to respiratory failure in cases of greater severity. Although it is a rare entity, it is associated with high mortality. Treatment is symptomatic and based essentially on hemodynamic and ventilatory support measures. The authors describe the case of a 37-year-old patient that develops a fulminant bilateral re-expansion pulmonary edema, associated with respiratory failure and hypovolemic shock after placement of thoracic drainage for decompression of spontaneous right pneumothorax.Key words: Reexpansion pulmonary edema, pneumothorax, hypovolemic shock, respiratory failure. IntroduçãoO Edema Pulmonar de Re-expansão (EPR) é uma complicação pouco frequente que pode surgir após o tratamento do colapso pulmonar secundário a derrame pleural, pneumotó-rax e atelectasia. A sua prevalência varia consoante as séries oscilando entre <1%-14%, em doentes sintomáticos 1,2 , até 29% em doentes assintomáticos, mas com alterações imagiológicas sugestivas 3 . Cursa com uma mortalidade de 20% 4 . A siopatologia do EPR não se encontra totalmente esclarecida. Contudo, pensa-se que alterações na microvasculatura pulmonar bem como alterações mecânicas decorrentes da rápida re-expansão possam estar implicadas. A hipoxémia no pulmão colapsado induz espessamento do endotélio e da membrana basal dos capilares pulmonares contribuindo para a diminuição da sua distensibilidade, favorecendo a destruição destes aquando da re-expansão. A re-expansão do pulmão previamente colapsado conduz à produção local de radicais livres e de mediadores in amatórios que contribuem para a lesão e aumento da permeabilidade vascular associada ainda à diminuição na actividade do surfactante 5 . ÖzetReekspansiyon pulmoner ödemi uzun süre kollabe kalmış akciğerin tedavisi sırasında ortaya çıkan, nadir ancak ölümcül olabilen bir komplikasyondur. Genellikle hava, sıvı veya atelektaziye sebep olan endobronşiyal lezyonun hız-lı bir şekilde boşaltılması veya uzaklaştırılması sonrası görülür. Pulmoner kollapsın 72 saatten uzun olması en önemli risk faktörüdür. Hızlı tanı ve uygun tedavi hayat kurtarıcıdır. Sonuç olarak bu çalışma ile reekspansiyon pulmoner ödemin tarihçesi, risk faktörleri, klinik bulguları, patofizyolojisi, tedavi yaklaşımları ve önleme yöntemleri literatür ışığında bir derleme ile sunmak amaçlandı. It generally occurs after rapid evacuation or removal, air, liquid or atelectasis caused by endobronchial lesion. Pulmonary collapse lasting for over 72 hours is most important risk factor. Rapid diagnosis and appropriate treatment is life saving. In conclusion, the history, risk factors, clinical presentation, pathophysiology, treatment and prevention methods of reexpansion pulmonary edema reported in the light of literatures with a review article in this study.
