Fatality rates and use of systemic thrombolysis in pregnant women with pulmonary embolism

Abstract: Aims Data on the early course and use of systemic thrombolysis in pregnant women with pulmonary embolism associated or not with haemodynamic failure are scarce. We investigated these aspects using the information from the German Nationwide Inpatient Registry (years 2005-2016). Methods and results In Germany, all diagnoses referring to hospitalized patients are coded according to the International Classification of Diseases and Related Health Problems, 10th Revision with German Modification. We analysed data of… Show more

“…PE remains a frequent cause of death in women aged 20–50 years. Younger women are characterised by an overall low mortality and present with specific risk factors for venous thromboembolism, such as pregnancy 63 and the use of oestrogen-containing oral agents, such as the combined oral contraceptive pill. In the USA, this peak appeared to extend to older women between 2000 and 2017, possibly reflecting the increase in the mean age of mothers for all birth orders observed during the same period.…”

“…In spite of 20% of women with cardiac arrest, 94% of women survived (95%CI 86-98%). This figure contrasts with a 57% survival among 67 pregnant women with PE treated with thrombolysis from a German administrative cohort [2]. Likely, the true survival lies between these numbers, which are affected by reporting bias and misclassification, respectively.…”

“…Its most severe form, high-risk ("massive") PE, causes hemodynamic instability through an acute extensive pulmonary arterial obstruction, and is associated with a 15-50% in-hospital mortality in non-pregnant populations [1]. During pregnancy, up to 7% of PEs may present with such hemodynamic instability [2], and high-risk pregnancy-associated PE (PA-PE) is responsible for about 10-15% of all maternal deaths in Europe and North America [3] [2].…”

“…Despite these reassuring trends, PE associated with hemodynamic instability portends unacceptably high rates of in‐hospital or early death 15,16 . Particularly vulnerable are women during pregnancy and postpartum and obese patients, for whom high‐quality evidence supporting clinical decisions is not available 17‐20 …”
