Favorable outcome in a case of Mycoplasma pneumoniae-associated crescentic glomerulonephritis

Abstract: Mycoplasma pneumoniae-associated nephritis has been reported in children with various pathological findings. It nevertheless remains an uncommon disease and, within this clinical context, endo-and extracapillary glomerulonephritis in a child has never been described. We report here a case of a 3-year-old girl diagnosed with severe crescentic glomerulonephritis associated with M. pneumoniae infection who presented with nephrotic syndrome and impaired renal function. The serum C3 complement level was initially l… Show more

“…Autoimmune reactions have been suggested to be responsible for many of these manifestations. In particular, M. pneumoniae -associated nephritis has been reported in few patients, with various pathological findings, including membranoproliferative GN, minimal change disease, endo-and extracapillary GN, and even immunoglobulin A nephropathy [5][6][7] . On the other hand, hemolytic anemia is recognized as a rare but severe complication of Mycoplasma infection.…”

“…Mycoplasma pneumoniae has been implicated as an etiology in various renal manifestations in both children and adults. Acute GN has been sporadically reported in association with M. pneumoniae infection [5][6][7] , while the combination of acute kidney failure and hemolytic anemia in the course of the infection has been character-…”

“…However, the organism has also been detected by PCR from a kidney biopsy specimen of a child with membranoproliferative glomerulonephritis who also had elevated IgM (263) and by immunoperoxidase staining of kidney tissue of a patient with acute interstitial cystitis (264). Renal complications can be of great clinical importance since they can lead to renal failure (265,266).…”

“…1 The identification of a previous case of acute postinfectious glomerulonephritis with transient hypocomplementemia (low serum C3 complement) associated with M. pneumoniae suggests transient complement activation. 15 The excessive activation of the alternative complement pathway results in endothelial cell injury and accumulation of the proinflammatory and procoagulant components that eventually trigger thrombosis. 1…”
