Feasibility and Reproducibility of Off-line Tissue Doppler Measurement of Regional Myocardial Function During Dobutamine Stress Echocardiography

Abstract: Quantification of myocardial functional reserve by off-line analysis of colour tissue Doppler acquired during dobutamine stress is feasible and reproducible in 11 segments of the left ventricle. The most reliable measurements are systolic velocities of longitudinal motion in basal segments.

“…TVI also plays an important role for the analysis of segmental LV contractility both at rest and during exercise or dobutamine stress [68], as it provides an objective quantification of regional ventricular function with good reproducibility in most LV segments [69]. A peak stress velocity response of [5.5 cm/s has been proposed to identify abnormal segments in all but apical LV regions with average 86% sensitivity and 81% specificity [70].…”

“…E’ velocity is easily measured with low inter‐observer variability 85 and has important prognostic value 81 , 86 . Decreased E’ is one of the earliest markers for DD and is present in all stages of the condition 87 , typically lowest in patients with restrictive filling.…”

“…Stress echocardiography remains, however, a semiquantitative approach using a scoring system for wall-motion abnormalities, which influences negatively inter-and intraobserver reproducibility [4]. Tissue Doppler echocardiography (TDE) quantifies wall motion velocity and has already been used during stress and exercise echocardiography [5,6]. A recently published study investigated in a large population of healthy neonates and children the influence of heart rate, body weight, and age on TDE derived parameter [7].…”
