Feasibility and Safety of Laparoscopic Complete Mesocolic Excision (CME) for Right-sided Colon Cancer

Abstract: The aim of this prospective clinical study is compare short-term outcome of laparoscopic right hemicolectomy using the Complete Mesocolic Excision (CME group) with patients who underwent conventional right-sided colonic resection (NCME group). Summary Background Data: Although CME with central vascular ligation in laparoscopic righthemicolectomy is associated with a significant decrease in local recurrence rates and improvements in cancerrelated 5-year survival, there may be additional risks associated with th… Show more

“…We performed a resection of a tract of 20 cm of medium ileum, including the site of perforation, using ultrasonic devices (Harmonic ACE+7 Shears with Advanced Hemostasis) and a linear surgical stapler (Echelon Flex 45 mm, white reload), followed by totally intracorporeal side-to-side mechanical anastomosis (Echelon Flex 45 mm, white reload). Stapler-access enterotomy was closed through double line continuous barbed suture (V-Loc 3–0, Medtronic) ( 33 ). Exploration of intestinal loops, from ileocecal valve to Treitz ligament, did not evidence further macroscopically detectable lesions.…”

“…A Veress needle was used to induce pneumoperitoneum in the left subcostal region ( 5 ) and the trocars were positioned as in left colectomy. The optical trocar was placed in the right para-umbilical region (2 cm from the median line) ( 6 ). Three additional trocars were inserted in the right iliac fossa (12 mm), in the right hypochondrium (5 mm), and in the left flank (5 mm).…”
