Feasibility of a novel dose fractionation strategy in TMI/TMLI

Abstract: BackgroundTo report our experience in planning and delivering total marrow irradiation (TMI) and total marrow and lymphatic irradiation (TMLI) in patients with hematologic malignancies.MethodsTwenty-seven patients undergoing bone marrow transplantation were treated with TMI/TMLI using Helical Tomotherapy (HT). All skeletal bones exclusion of the mandible comprised the treatment target volume and, for TMLI, lymph node chains, liver, spleen and/or brain were also included according to the clinical indication. “…Dose homogeneity at the junction region from the overlap of the two plans was considered in the treatment planning stage [22]. In the delivery stage, we have performed three MVCT scans in Plan-upper and one for Plan-lower [22], but no scans covered the dose junction region, after couch shifts of the fourth MVCT scans, the three consecutive slices (used for section plan dose junction) from the Plan-upper and Plan-lower may mismatch. Resulting in hot-spots and cold-spots near the junction region.…”

“…1a, all patients were immobilized using a home-made dedicated immobilization system. CT scans, treatment planning and delivery details can be found in our previous study [22].

“…This procedure can be very time-consuming and prolong the overall treatment time. Similarly, Bao et al [16] used a home-made immobilization system with integrated vacuum cushions and three thermoplastic masks for patient positioning. The authors describe that this system was turned 180° by two therapists into a FFS position.…”

“…Helical Tomotherapy (HT) as a new CT-image guided rotational intensity modulated radiotherapy can deliver highly conformal dose distributions [7,8]. To large complex target shapes while simultaneously avoiding doses to critical normal organs, making it an attractive option for the delivery of conformal targeted TBI [9]. Total bone marrow and involved lymphoid irradiation (TMLI) was designed to treat bone marrow and involved targets, including lymph node chains, liver, spleen, brain, spinal cord and testes [9,10].…”

The potential advantages, the acute toxicities, initial clinical experiences, and challenges of this approach were reported recently [10] .…”
