Feasibility of a novel laparoscopic technique with unidirectional knotless barbed sutures for the primary closure of duodenal ulcer perforation

Abstract: The use of a continuous suture technique with unidirectional barbed sutures is as safe as the conventional suture technique and allows easier and faster suturing in the repair of perforated peptic ulcers.

“…However, in previous studies, operating times have been reduced from an average of 131 minutes to 87 minutes by performing continuous suturing using a barbed suture. 16 Thus, the present authors saved time for performing additional HSV procedures on patients in the high-risk group for ulcer recurrence. Both surgeons in the present study reduced the mean operation time to less than 100 minutes after performing the procedure in 10 cases each, suggesting that the procedure would be easy to implement if a surgeon is familiar with laparoscopic gastrectomy techniques.…”

“…The detailed laparoscopic primary procedure, performed with a barbed suture at the present authors’ centre, has been described previously. 16 There was no risk of additional contamination after suturing of the perforation, and intraperitoneal irrigation was performed. Between 5 000 and 10 000 cc of sterile warm normal saline was used to clean the abdominal cavity.…”
