Feeling of presence in Parkinson's disease

Abstract: Although FP is not a sensory perception, projection of the sensation into the extrapersonal space, along with the frequent co-occurrence of elementary visual hallucinations and the strong association with visual hallucinations or illusions, supports its hallucinatory nature. FP may be viewed as a 'social' hallucination, involving an area or network specifically activated when a living being is present, independently of any perceptual clue.

“…We choose to integrate 'guardian angel' item in PSAS because it is a projection of the sensation into the extrapersonal space, along with the frequent co-occurrence of elementary visual hallucinations and the strong association with visual hallucinations or illusions, supporting its hallucinatory nature (Fenelon et al, 2011). Moreover, 'guardian angel' sensations were recognized as common in PD (Sanchez-Ramos et al, 1996) and even described in SCZ (Koehler and Sauer, 1984), and probably underestimated because they were not explored.…”

“…A specific item on 'guardian angel', already well described in PD by Fenelon, was added (Fenelon et al, 2011). It defined this feeling of presence to the vivid sensation that somebody is present nearby, when no one is actually there, in the absence of sensory clues revealing a presence.…”

“…Relatedly, RBD in PD or LBD is associated with a lower Braak stage and independently predicts the development of hallucinations (Arnulf, 2013; Dugger et al, 2012). In the early stages of PD 20 to 45% of patients report fleeting sideway shadows (‘passage’ hallucinations) or a sensation of presence (Fénelon, Soulas, Cleret de Langavant, Trinkler, & Bachoud-Levi, 2011; Fénelon, Soulas, Zenasni, & de Langavant, 2010; Mack et al, 2012). Pareidolias and visual hallucinations are common in patients with LBD (Uchiyama et al, 2012).…”

“…[567] In addition, there are only limited data on the pattern of psychosis in PD, especially from India. [8] Therefore, the present study was undertaken to study the clinical profile of the patients with Parkinson's disease and psychosis using the neuropsychiatric inventory (NPI) for rating of psychotic symptoms.…”
