Fenofibrate As an Adjuvant to Phototherapy in Term Neonates with Hyperbilirubinemia; A Randomized Controlled Clinical Trial

Abstract: Background: Neonatal jaundice results from accumulation of bilirubin as fetal hemoglobin is metabolized by the immature liver. It may be physiological or pathological. Although most infants recover without major morbidity, yet some might develop sever hyperbilirubinemia and acute bilirubin encephalopathy as result of high levels of serum bilirubin. Fenofibrate is a member of the fibrates group. It is a safe and inexpensive orally administered fibric acid derivative conventionally used to treat dyslipidemia. In… Show more

“…Many previous RCTs have evaluated fenofibrate as an adjunctive treatment to phototherapy in neonates with elevated levels of indirect bilirubin [94][95][96][97]. Fibrate is a class of phenoxy-isobutyric acid derivative, which consists of peroxisome proliferator-activated alpha receptor agonists, used as a lipid-lowering drug that can also enhance bilirubin conjugation and excretion through the induction of UGT1A1.…”

“…Fibrate is a class of phenoxy-isobutyric acid derivative, which consists of peroxisome proliferator-activated alpha receptor agonists, used as a lipid-lowering drug that can also enhance bilirubin conjugation and excretion through the induction of UGT1A1. A group of researchers reported that a single dose of fenofibrate at a dose of 10 mg/kg during phototherapy in newborns is associated with a significant reduction in serum bilirubin levels [94]. Another study failed to demonstrate a statistically significant difference in the rate of bilirubin reduction despite the shorter duration of phototherapy [98].…”

“…Fibrate, as a lipid-lowering drug, can enhance bilirubin conjugation and excretion through the induction of UGT1A1 (12). Fibrates are a class of phenoxy-isobutyric acid derivatives, which are peroxisome proliferator-activated alpha receptor agonists.…”

“…Additionally, clofibrate processes the transfer of albumin-bound bilirubin to hepatocytes and induces hepatic uptake and conjugation of bilirubin (13). Numerous RCTs assessed fenofibrate as an adjuvant to phototherapy in neonates with IHB (12)(13)(14)(15)(16). Khafaga et al revealed that a single dose of fenofibrate (10 mg/kg) as an adjuvant to phototherapy in neonates with hyperbilirubinemia was associated with a significant reduction of serum bilirubin levels (12).…”
