Fertility after intact ovary transplantation

Abstract: Transplantation surgery, which is limited by the supply and short-term viability of fresh donor organs, would be revolutionized if these could survive freezing, but early claims of cryopreservation were never realized. Here we describe the successful transplantation in rats of ovaries, fallopian tubes and the upper segment of the uterus en bloc after storage in liquid nitrogen.

“…Transplantation of the whole ovaries by vascular anastomosis has been performed in experimental animals [11,19,20]. Moreover, fertility in these animal transplantation models was evidenced by the delivery of offspring by the experimental rats and ewes [11,21]. Despite of these encouraging results, however, a dramatic depletion in follicular density was observed in transplanted frozenthawed grafts.…”

“…In the past few years, attempts at freezing and grafting whole ovaries in animal have yielded encouraging results. The first case of restoration of fertility in rats after whole frozen-thawed ovary transplantation was described by Wang et al in 2002 [13]. They described successful vascular transplantation of frozen-thawed rat ovaries and reproductive tract in 4 out of 7 (57 %) transplants.…”

“…Spontaneous pregnancies after transplantation of an intact ovary with microvascular anastomosis have occurred in rat, ewe, and human [12][13][14]. It shows that this technique is promising.…”

“…One of the future approaches to preserve fertility is the transplantation of an intact ovary with vascular anastomosis, allowing immediate revascularization of the ovary. Transplantation of fresh whole ovary with vascular anastomosis has already been successfully performed in rats, 71 rabbits, 72 sheep, 73 dogs, 74 monkeys 75 and humans. 76 The first case of restoration of fertility after whole frozen ovary transplantation was reported by Wang et al in 2002.…”
