Fetal Bowel Dilatation: A Sonographic Sign of Uncertain Prognosis

Abstract: Fetal bowel dilatation is an indirect sonographic sign of mechanical or functional bowel obstruction. The etiology of fetal bowel dilatation is a difficult prenatal diagnosis since ultrasound has limited accuracy for bowel evaluation. The authors describe a case of fetal bowel dilatation diagnosed in the third trimester.

“…Different prognostic factors were identified: (1) the presence or not of associated malformations, (2) the result of complementary tests in amniotic fluid, (3) the progression of dilatation between two US examinations, and (4) MRI to confirm the level of obstruction. These prognostic factors have been described in the literature but frequently independently, 15–22 and we propose a policy to improve the management of pregnancies.…”

“…There are multiple reasons, functional and mechanical, why fetal bowel obstruction occurs, such as congenital disorder, intestinal malrotation/volvulus, and even meconium plug syndrome. 2,3 Identifying the etiology, nonetheless, remains difficult due to the limited accuracy of ultrasound imaging. As such, this requires prompt attention from the neonatal specialists as well as pediatric surgeons and careful work-up is warranted in order to devise a proper treatment.…”

“…In our case, maternal ultrasound up to the 29 th week was reported as normal but routine third trimester ultrasound showed dilated bowel loops. Dilated bowel loops are highly indicative of bowel obstruction due to a congenital disorder, intestinal malrotation, volvulus, and meconium plug syndrome (13).…”
